Cardiac imaging on the multidetector computed tomography

Abstract:

Tato závěrečná bakalářská práce je zaměřena na vyšetření srdce na multidetektorové výpočetní tomografii. Speciálně na vyšetření koronárních tepen, jejichž uzávěr je nejčastěji důvodem nekrotizace myokardu. V minulých letech toto vyšetření plně zastupovala digitální substrakční angiografie, nyní patří koronarografie počítačovou tomografií k nejmodernějším a nejefektivnějším metodám vyšetření srdce. Toto vyšetření pacienta zatěžuje minimálně jak z hlediska množství podané kontrastní látky, tak i časově, jelikož probíhá ambulantně. Práce je rozdělena na část teoretickou a na studii. Teoretická část práce je zaměřena na vývoj počítačové tomografie, technické parametry důležité k vyšetření myokardu, dále také na anatomii a patologii srdce, vyšetření pomocí kontrastních látek a na využití moderních postprocesingových metod. Cílem studie bylo zjistit, jaká je struktura pacientů přicházejících k vyšetření, jaké jsou nejčastější indikace ke koronarografii. Dále bylo v mém zájmu zjistit, jaké vyšetření koronarografii předcházelo, v jakém množství byla použita kontrastní látka a jaká byla výsledná diagnóza. Studie probíhala na radiologickém oddělení ve Vojenské nemocnici v Olomouci. Výsledkem je zjištění, že vyšetření koronárních arterií jsou častěji vystavení muži a to ve věku 51- 60 let. Bylo zjištěno, že u všech pacientů byla předem provedena elektrokardiografie a echokardiografie. Mezi jednu z nejčastějších diagnóz patří bolest na hrudi. Cílem bylo zjistit, jak byla metoda přínosná a jaká je struktura pacientů přicházejících k vyšetření. Nejčastěji byly potvrzeny aterosklerotické změny a stenózy na koronárních arteriích.

Abstract:

This final work focuses on the examination of heart using the multidetector computed tomography. The main goal of the thesis is the investigation of the coronary arteries whose closure is the most common reason for the necrosis infarction. In past years was this kind of examination represented mainly by the fully digital subtraction angiography, but nowadays the coronary angiography is one of the most modern and effective methods of heart examination. It burdens the patient the least, in terms of both quantity of contrast agent administered and the time spent, because it can be done ambulatory. The thesis is divided into the theoretical part and the study. The theoretical part is aimed at the development of the computed tomography, technical parameters important for the investigation of the heart attack, also anatomy and pathology of heart and at the examination using contrast agents and the use of modern postprocessing methods. The aim of this study was to ascertain the structure of patients coming for examination together with the most common indications for coronary angiography. It was also in my interest to find out what testing preceded the coronary angiography; in what quantity the contrast agent was used and what was the final diagnosis. The study took place at the radiological department at the Military Hospital in Olomouc. One of the conclusions was that men in the age of 51 - 60 years are more susceptible to the coronary arteries. Also, it was found that all patients had previously undergone through electrocardiography and echocardiography. The most common diagnoses include chest pain. Another goal was to determine how the method was beneficial and what the structure of patients coming for the examination was. The most frequently atherosclerotic changes and stenosis of coronary arteries were confirmed.
