Analysis of the intestinal microbiome and determination of gluten peptides in patients with celiac disease

Abstract:

Celiakie je s celosvětovou prevalencí 1–2 % jedním z nejčastějších autoimunitních onemocnění. Postihuje geneticky predisponované jedince bez ohledu na vek, pohlaví či etnicitu. Jediným řešením pro jedince diagnostikované s celiakii je přísná, celoživotní bezlepková dieta. Jako hlavní genetické determinanty predisponující jedince k celiakii byly identifikovány haplotypy HLA-DQ2 a HLA-DQ8, ale samotná přítomnost těchto alel plně nepodněcuje vývoj onemocnění. Pro rozvoj celiakie je potřebná souhra genetické složky, faktorů životního prostředí a konzumace lepku, který se nachází v různých obilovinách včetně pšenice, ječmene a žita. Lepek neboli gluten, je složen z gluten imunogenních peptidů (GIPs), které při tráveni lepku, představují klíčový spouštěč imunitní odpovědi a následného poškození tkáně ve střevě. Výzkumy dokazují, že důležitou roli v patogenezi celiakie hrají také environmentální faktory, jako je například střevní mikrobiom. Ve střevech se nachází více než 70 % imunitního systému a změny ve složení či rozmanitosti střevního mikrobiomu můžou modulovat imunitní odpověď a podílet se na patogenezi celiakie. Diplomová práce se zaměřuje na detekci gluten imunogenních peptidů v různých vzorcích, jako je stolice a moč, pacientů s celiakií jako metoda sledování dodržování bezlepkové diety. Pro lepší pochopení onemocnění byl analyzován střevní mikrobiom a porovnány rozdíly ve složení mezi pacienty s celiakií a kontrolní skupinou.

Abstract:

With a global prevalence of 1-2%, celiac disease is one of the most common autoimmune diseases. It affects genetically predisposed individuals regardless of age, sex, or ethnicity. The only solution for individuals diagnosed with celiac disease is a strict, lifelong gluten-free diet. HLA-DQ2 and HLA-DQ8 haplotypes have been identified as the main genetic determinants predisposing individuals to celiac disease, but the presence of these alleles alone does not fully promote disease development. The development of celiac disease requires a combination of genetics, environmental factors and the consumption of gluten, which is found in various cereals including wheat, barley and rye. Gluten is composed of gluten immunogenic peptides (GIPs), which, when digested, together with the genetic component, are a key trigger of the immune response and subsequent tissue damage in the gut. Research shows that environmental factors, such as the gut microbiome, also plays an important role in the pathogenesis of celiac disease. More than 70 % of the immune system is located in the gut and changes in the composition or diversity of the gut microbiome may modulate the immune response and contribute to the pathogenesis of celiac disease. This thesis focuses on the detection of gluten immunogenic peptides in various specimen, such as stool and urine, of patients with celiac disease as a method to monitor adherence to a gluten-free diet. To better understand the disease, the gut microbiome was analysed and the differences in composition between celiac and control group were compared.
