Life-threatening Peripartum Haemorrhage from the Midwife's Perspective

Abstract:

Life-threatening Peripartum Haemorrhage is a serious complication during childbirth that can have fatal consequences for both, the mother and the fetus. This bachelor‘s thesis focuses on the perspective of the midwife on peripartum bleeding, its risk factors, prevention, causes, diagnosis, management of the situation and organizational principles. The aim of this thesis is to highlight the area of care for woman with such a complication. The teoretical part describes hematological changes in pregnancy, hemostasis, the most common causes of excessive antepartum, intrapartum and postpartum bleeding, issues related to peripartum life-threating haemorrhage, epidemiology, risk factors, etiology, its management and organizational principles. The practical part consists of a case study of a patient with this serious complication during her childbirth. The case report includes anonymized medical history, the course of pregnancy, delivery and a description of the life – threatening peripartum haemorrhage, the elimination of its cause and the subsequent post – operative care, until the patient’s discharge to the home enviroment, where the main goal is to return the patient to normal life as much as possible.

Abstract:

Peripartálne život ohrozujúce krvácanie je vážna komplikácia v čase pôrodu, ktorá môže mať fatálne následky pre matku a plod. Táto bakalárska práca sa zameriava na pohľad pôrodnej asistentky na peripartálne krvácanie, jeho rizikové faktory, prevenciu, príčiny, diagnostiku, manažment vzniknutej situácie a organizačné zásady. Cieľom tejto bakalárskej práce je poukázať na oblasť starostlivosti o ženu s takouto komplikáciou. Teoretická časť popisuje hematologické zmeny v tehotenstve, hemostázu, najčastejšie príčiny nadmerného antepartálneho, intrapartálneho a postpartálneho krvácania, problematiku peripartálneho život ohrozujúceho krvácania, jeho epidemiológiu, rizikové faktory, etiológiu, manažment, diagnosticko – liečebné postupy a organizačné zásady. Praktická časť sa skladá z kazuistiky pacientky s touto vážnou komplikáciou, ktorá nastala počas jej pôrodu. Kazuistika obsahuje anonymizované anamnestické údaje, priebeh tehotenstva, pôrodu a popis vzniknutého peripartálneho život ohrozujúceho krvácania, odstránenie jeho príčiny, následnú pooperačnú starostlivosť, až do prepustenia pacientky do domáceho prostredia, kde hlavným cieľom je návrat pacientky do bežného života v čo najväčšej možnej miere.
