Long-Term Hospitalization of a Pregnant Woman with Premature Amniotic Fluid Leakage from a Midwife's Perspective

Abstract:

Tématem předložené bakalářské práce je snaha o uchopení a přiblížení komplexní problematiky, jakou předčasný odtok plodové vody a s ním spojená hospitalizace těhotné ženy, často dlouhodobá, bezpochyby je. Práce je rozdělena na dvě části. Teoretická část se snaží za pomoci odborné literatury a doporučených postupů popsat problematiku předčasného předtermínového odtoku plodové vody, přiblížit problematiku hranice viability plodu a zmapovat management tohoto klinického stavu. Praktická část formou kazuistik přináší vhled do problematiky předčasného odtoku plodové vody a stručně také do dimenze viability. Ukazuje průběh dlouhodobé hospitalizace těhotné ženy a péči o ni. Prezentuje, že se jedná o interdisciplinární problém, který v sobě sdružuje řadu diagnóz, a přibližuje pohled na možnosti moderní diagnostiky a klinického managementu oddálení předčasného porodu. Ze závěrečného porovnání vychází, že i přesto, že klinický management reflektuje nejaktuálnější doporučené postupy, ne vždy je úspěšný ve smyslu vystoupení z pásma těžké nezralosti novorozence. Ačkoliv se u jedné z pacientek nepodařilo předčasnému porodu zabránit, pozitivním důsledkem léčby u obou pacientek bylo oddálení porodu natolik, aby mohla proběhnout kúra kortikosteroidy a antibiotiky. Podařilo se dosáhnout maturace plic obou plodů a zabránit rozvoji systémové zánětlivé odpovědi.

Abstract:

The topic of the submitted bachelor's thesis is the effort to grasp and approach the complexity in an issue, such as premature pre-term amniotic fluid leakage and the associated hospitalization of a pregnant woman, often long-term, is undoubtedly. The thesis is divided into two parts. With the help of professional literature and recommended procedures the theoretical part tries to describe the issue of premature pre-term amniotic fluid leakage, to describe the issue of fetal viability and to explore management options for this clinical condition. In the form of two case studies this thesis provides insight into the issue of premature amniotic fluid outflow and briefly into the dimension of viability. It shows the course of long-term hospitalization of a pregnant woman and provided medical care. Premature amniotic fluid leakage is an interdisciplinary problem, which combines a number of diagnoses, and brings a closer look at the possibilities of modern diagnostics and clinical management of preterm birth. The final comparison shows that even though clinical management reflects the most current recommenden procedures, it is not always successful in terms of leaving the newborn's severe immaturity zone. Although failure of the treatment of one of the patients in delaying preterm birth, in both patients the delivery was delayed long enough to allow applying corticosteroid and antibiotic treatments. Pulmonary maturation of both fetuses was achieved and systemic inflammatory responses were prevented.
