Economic analysis of patient care after lower limb amputation from perspective of the healthcare provider

Abstract:

Diplomová práce Ekonomická analýza péče o pacienty po amputaci dolní končetiny z pohledu poskytovatelů zdravotní péče si klade za cíl vyčíslit náklady na péči o tyto pacienty pro jednotlivé odbornosti a následně stanovit orientační celkové náklady na péči. První podkapitola teoretické části je zaměřena na problematiku amputací a klade si za cíl popsat současný algoritmus péče o pacienty po amputaci dolní končetiny. Druhá podkapitola teoretické části uvádí přehled nejčastěji využívaných typů ekonomických analýz. Druhá podkapitola je dále zaměřena na popis vypracování Cost-of-illness analýzy. Třetí podkapitola teoretické části popisuje systém vykazování zdravotní péče v České republice. Metodika práce stanovuje parametry vypracování práce. Diplomová práce byla vypracována typem ekonomické analýzy Cost-of-illness typem top - down. Tento typ analýzy předpokládá sběr dat od poskytovatelů zdravotní péče, analýzu statistických dat a analýzu související legislativy a dalších volně přístupných materiálů. Data od poskytovatelů byla sbírána formou rozhovorů. Časové období, které tato diplomová práce u pacientů zkoumá je vymezeno od provedení operačního výkonu do výroby a aplikace primární protézy. V metodice jsou rovněž uvedeny otázky, které byly kladeny poskytovatelům zdravotní péče během rozhovoru. Kapitola Výsledky je věnována zpracování dat získaných z rozhovorů a jejich komparaci s platnou legislativou, statistickými daty a dalšími daty z volně dostupných zdrojů. Jsou zde vyčísleny náklady pro jednotlivé typy poskytovatelů. Na konci kapitoly výsledky jsou orientačně vyčísleny celkové náklady za péči poskytnutou pacientům po amputaci dolní končetiny. Celá práce je shrnuta v kapitole závěr. Skutečnosti zjištěné tvorbou této diplomové práce jsou následně porovnány se zahraničními prameny v kapitole Diskuse.

Abstract:

The diploma thesis Economic Analysis of Care for Lower Limb Amputees from the Perspective of Health Care Providers aims to quantify the cost of care for these patients for individual specialisations and then determine the approximate total cost of care. Focusing on the issue of amputations, the first subchapter of the theoretical part aims to describe the current algorithm of care for lower limb amputees. The second subchapter of the theoretical part gives an overview of the most commonly used types of economic analyses. The second subchapter also deals with the description of making a cost-of-illness analysis. The third subchapter of the theoretical part describes the health care reporting system in the Czech Republic. The methodology of the thesis sets out the parameters for writing the thesis. The diploma thesis has been written using the top-down type of the cost-of-illness economic analysis. This type of analysis assumes the collection of data from health care providers, analysis of statistical data, and analysis of related legislation and other freely available materials. Data from the providers was collected through interviews. The time period examined by the thesis in patients is defined as the time from surgery to the manufacture and application of the primary prosthesis. The methodology also includes the questions given to the health care providers during the interviews. The Results chapter focuses on the processing of the data obtained from the interviews and the comparison of the data with current legislation, statistical data and other data from freely available sources. It also quantifies cost for individual providers. At the end of the Results chapter, the total cost of care provided to lower limb amputees is tentatively quantified. The entire thesis is summarised in the Conclusion chapter. The findings of the thesis are then compared with foreign sources in the Discussion chapter.
