The importance of thermoregulation in the perioperative and early postoperative care

Abstract:

Cíl: Cílem práce bylo zhodnotit snahu o udržení normotermie v perioperační péči na operačních sálech ve Fakultní nemocnici v Ostravě. Předmětem mého zájmu bylo kontinuální měření tělesné teploty u pacientů během operačního výkonu při otevřené laparotomii v celkové anestezii. Metodika: Metodou bylo měření tělesné teploty u pacientů během perioperačním období podstupujících operační zákrok otevřenou laparotomií v celkové anestezii. Použití termických pomůcek bylo ponecháno na zvyklostech pracoviště a kontinuálním měřením tělesné teploty bylo zjišťováno, zda dochází k poklesu teploty během operace, zda ohřívací techniky dostatečně zabraňují nežádoucí hypotermii. Pro dosažení výzkumných cílů a následného ověření hypotéz byla zvolena kvantitativní metoda sběru dat, strukturované pozorování pomocí měření tělesné teploty. Teplota byla měřena kontinuálně pomocí jícnového čidla zavedeného do distální části jícnu. Součástí měření bylo sledování krevních ztrát během operace, přítomnost svalového třesu po odeznění celkové anestezie, celková délka operačního výkonu i celková délka anestezie, BMI, použité termické pomůcky a teplota operačního sálu. Výzkum probíhal na operačních sálech Fakultní nemocnice s poliklinikou v Ostravě a byl schválen a povolen náměstkyní ředitele pro ošetřovatelskou péči. Pro testování hypotéz byl použit Fisherův exaktní test a Mann-Whitneyův test pro dva výběry Výsledky: Vyhodnocená měření kontinuální tělesné teploty nebyla statisticky významná, nicméně se ukázalo, že metody ohřevu pacientů při operačních výkonech jsou ve FNO dostačující a ošetřující personál dbá na tepelný komfort pacientů. Závěr: Cílem práce bylo zhodnotit snahu o udržení normotermie na operačních sálech FNO. Výsledné šetření ukázalo, personál FNO má snahu o udržení tepelného komfortu pacientů, využívá několik metod ohřevu samostatně nebo v kombinacích. Bohužel stále není standardem kontinuální měření TT a proto není případná hypotermie zcela pod kontrolou.

Abstract:

Objective: The aim of this work was to assess efforts to maintain normothermia in the perioperative care on operating rooms at The University Hospital in Ostrava. The subject of my interest was continuous measuring of the patients´ body temperature during surgery with an open laparotomy under general anesthesia. Methods: The method was measuring the body temperature of patients during the perioperative period undergoing surgery by open laparotomy under general anesthesia. Using thermal equipment was left on workplace practices and by continuous measurement of body temperature it was examined whether there is a drop in temperature during operation and whether the heating techniques prevent enough undesirable hypothermia. To achieve the research objectives and the subsequent verification of hypotheses, a quantitative method of data collection was chosen, structured observation by measuring body temperature. The temperature was measured continuously by means of the esophageal probe inserted in the distal part of the esophagus. Part measurements were monitoring blood loss during surgery, presence of shivering after recovery of general anesthesia, the total length of surgery and general anesthesia duration, BMI, used thermal equipment and the temperature of the operating room. The research was conducted in operating rooms of The University Hospital in Ostrava and was approved and authorized by Deputy Director for Nursing Care. The Fisher's exact test and the Mann-Whitney test for two teams were used to test the hypotheses. Results: Evaluating continuous measurement of body temperature was not statistically significant, however it turned out that the heating methods of patients during surgery are sufficient in The University Hospital and nursing staff cares for thermal comfort of patients. Conclusion: The aim was to assess efforts to maintain normothermia in operating rooms of The University Hospital. The resulting investigation revealed that the staff tends to maintain thermal comfort of patients, it uses several methods of heating alone or in combinations. Unfortunately there is still no standard continuous measurement TT and that´s why the possible hypothermia is not completely under the control.
