Dynamic parameters of gait in patients after total hip arthroplasty

Abstract:

Cílem práce bylo zhodnotit vliv totální endoprotézy kyčelního kloubu na zatížení nohy (velikost tlaku a jeho rozložení na kontaktu nohy s podložkou) při chůzi. Teoretická část shrnuje poznatky o základní anatomii a kineziologii kyčelního kloubu, zabývá se problematikou osteoartrózy a jejími rizikovými faktory, náhradami kyčelního kloubu, chůzí a její analýzou. Praktická část prezentuje a hodnotí výsledky testování rozdílů mezi operovanou a neoperovanou dolní končetinou, v době před operací a po operaci. Velikost zatížení nohy jsme hodnotili pomocí doby kontaktu oblastí plosky nohy vzhledem k celkové době kontaktu během krokového cyklu, tlakového impulsu, doby dosažení maximálního tlaku a maximální hodnoty tlaku v dané oblasti. Výzkumný soubor tvořilo 10 probandů (4 muži, 6 žen, průměrný věk 66?15,8 let, výška 168,1?11,5 cm, hmotnost 80,4?16,5 kg), kteří byli pacienty Ortopedické kliniky Fakultní nemocnice v Olomouci. K analýze zatížení nohy při chůzi byla použita plantografická plošina Footscan?. Na operované dolní končetině a neoperované dolní končetině jsme nalezli minimální rozdíly v zatížení jednotlivých oblastí plosky nohy v porovnání před a po operaci. S výjimkou doby kontaktu a tlakového impulsu v oblasti hlavičky 2. metatarsu, kdy na operované dolní končetině došlo po operaci ke snížení hodnot, byly všechny zbývající rozdíly nevýznamné na hladině p < 0,05.

Abstract:

The aim of this study was evaluation of the total hip replacement to weigh-bearing of the foot (the pressure level and it´s distribution on the foot sole while in contact with the ground) during walking. The theoretical part summarizes findings of the basic anatomy, kinesiology of the hip joint, osteoarthritis and it´s risk factors, hip replacements, walking and gait analysis. The practical part presents and evaluates results of testing the differences between the operated and non-operated lower limb, before and after the operation. The weigh-bearing of the foot was evaluated by using contact time of areas on the foot sole with respect to the total contact time during the gait cycle, the pressure pulse, the time of maximum pressure and the maximum pressure in the area of foot sole. The sample consisted of 10 probands (4 men, 6 women, average age 66?15,8, height 168,1?11,5 cm, weight 80,4?16,5 kg) who were patients of the Orthopedic Clinic of the University hospital in Olomouc city. To analyze the weigh-bearing of the foot sole while walking we used a plantography platform Footscan?. We found minimal differences in the weigh-bearing of each areas of the foot sole in comapare with before and after surgery operated and non-operated lower limbs. With the exception of the contact time and the pressure impulse in the second metatrsal where a reduce of values on the operated lower limb has occurred but all differences were at an insignificant level p < 0,05.
