Rehabilitation protocol for a patient with Miller-Fisher Syndrome: a clinical case

Abstract:

In 1932 James Collier described a syndrome characterized by ataxia during walking, ophthalmoplegia and areflexia and in 1956 Charles Miller Fisher recognized this syndrome, called Miller Fisher Syndrome (SMF), as a variant of Guillain-Barr? Syndrome, which affects the cranial nerves. In this paper, Miller Fisher Syndrome is presented with its onset, the clinical picture, the pathological mechanisms, the trasmission, its incidence and the main differences with the Guillain-Barr? syndrome. The objective of this thesis is to describe the rehabilitation methods applied to a patient affected by Miller Fisher Syndrome, underling the importance and role of the rehabilitation approach. In particular, the Bobath method and the Kabat method were taken into consideration: the first allowed to obtain excellent results as regards the recovery of the trunk and the second proved to be extremely effective for the re-education of the facial nerve. The results were quantified through the use of some functional evaluation scales. From the comparison of the results obtained, it can easily be seen that there is a progressive and global improvement in the patient's condition. In conclusion, the results achieved at the end of the rehabilitation path make the two methods used a valid possibility for the tratment of Miller Fisher Syndrome. Infact, a patient undergoing specific and progressive rehabilitation treatments can restore full autonomy.

Abstract:

Nel 1932 James Collier descrisse una sindrome caratterizzata da atassia durante la deambulazione, da oftalmoplegia e da areflessia e nel 1956 Charles Miller Fisher riconobbe tale sindrome, detta appunto Sindrome di Miller Fisher (SMF), come una variante della Sindrome di Guillain-Barr?, che interessa i nervi cranici. In questo elaborato, viene presentata la Sindrome di Miller Fisher con il suo esordio, il quadro clinico, i meccanismi patologici, la trasmissione, la sua incidenza e le principali differenze con la sindrome di Guillain-Barr?. L'obiettivo di questa tesi ? quello di descrivere le metodiche riabilitative applicate ad una paziente affetta dalla Sindrome di Miller Fisher, sottolinenando l'importanza ed il ruolo dell'approccio riabilitativo. In particolare si ? presa in considerazione la metodica Bobath e la metodica Kabat: la prima ha permesso di ottenere ottimi risultati per ci? che riguarda il recupero del tronco e la seconda si ? dimostrata di estrema efficacia per la rieducazione del nervo faciale.I risultati sono stati quantificati attraverso l'utilizzo di alcune scale di valutazione funzionale. Dal confronto dei risultati ottenuti, si pu? facilmente notare come ci sia un progressivo e globale miglioramento delle condizioni del paziente. In conclusione i risultati raggiunti alla fine del percorso di riabilitazione, rendono le due metodiche utilizzate una valida possibilit? per il trattamento della Sindrome di Miller Fisher. Infatti un paziente sottoposto a trattamenti riabilitativi specifici e progressivi pu? ripristinare una piena autonomia.
