Functional endonasal endoscopic surgery - educational process of patients

Abstract:

Cíl: Cílem této bakalářské práce bylo vypracování edukačního plánu a vytvoření edukačního materiálu pro pacienty po funkční endonazální endoskopické chirurgii. Teoretická část bakalářské práce obsahuje anatomii a patologii horních cest dýchacích. Dále je zde popisována charakteristika výkonu, indikace k operaci, rizika operace, předoperační a pooperační přípravu pacienta, nežádoucí komplikace. V jedné části bakalářské práce je vysvětleno, co je edukace, a jaké má fáze. Poté jsou zde vysvětleny základní pojmy edukace. V další části je vytyčena ošetřovatelská péče o pacienta před operací, po operaci a péče pacienta doma. Praktická část je zaměřená na samotný edukační proces pacienta. Edukace probíhala individuálně s každým pacientem na pokoji. Probíhala v rámci dvou na sobě nezávislých rozhovorů. První edukace probíhala den před operací při příjmu pacienta na oddělení. Druhá edukace proběhla ve stejný den odpoledne. Hlavním cílem bylo zjistit, v jakých oblastech ošetřovatelské péče mají pacienti nedostatečné informace. Dílčím cílem bylo vytvoření edukační brožury, kterou mohli pacienti využívat po celou dobu hospitalizace a následně v domácím prostředí. Edukační materiál byl vypracován pro pacienty jako pomůcka k rozšíření jejich základních vědomostí, zlepšení jejich zdravotního stavu a předcházení nežádoucích pooperačních komplikací. Edukační materiál obsahuje indikace k operaci, správnou péče o operační ránu, možné nežádoucí komplikace, předcházení jim a jejich příznaky. Dále v nich pacient nalezne možné pomůcky ke zlepšení aplikace nosních roztoků. Edukováni byli čtyři pacienti na oddělení ORL, chirurgie hlavy a krku. Výsledky: Edukováni byli čtyři pacienti na oddělení ORL, chirurgie hlavy a krku. Pomocí vstupního dotazníku a rozhovoru s jednotlivými pacienty byly stanoveny ošetřovatelské diagnózy a sestaven edukační proces. Nejvíce neznalosti měli pacienti v rámci pooperační péče o ránu a o komplikacích. Pacienti sdělili, že během hospitalizace využili informace z vytvořené brožury. Všichni pacienti k edukaci přistupovali aktivně a shodli se, že pro ně edukace byla přínosem a obohacením stávajících vědomostí. Závěr: Z pilotních výsledků edukace bylo zjištěno, že pečlivá edukace pacientů při FESS je důležitá pro pooperační péči. Pro praxi bych doporučila vymezit dostatek času a klidného prostoru pro edukaci pacientů.

Abstract:

Aim: The aim of this bachelor thesis was to develop an educational plan and educational material for patients after functional endonasal endoscopic surgery. The theoretical part of the bachelor thesis includes anatomy and pathology of the upper respiratory tract. It also describes the characteristics of the procedure, indications for surgery, risks of surgery, preoperative and postoperative preparation of the patient, adverse complications. In one part of the bachelor's thesis it is explained what education is, and what stages it has. Then the basic concepts of education are explained. In the next part, the nursing care of the patient before surgery, after surgery and the care of the patient at home is outlined. The practical part is focused on the patient education process itself. The education took place individually with each patient in the room. It was conducted in two independent interviews. The first education took place the day before the operation when the patient was admitted to the ward. The second education took place on the same day in the afternoon. The main aim was to find out in which areas of nursing care patients have insufficient information. A sub-objective was to create an educational booklet that patients could use throughout their hospitalization and subsequently at home. The educational material was developed for patients as an aid to increase their basic knowledge, improve their health status and prevent unwanted postoperative complications. The educational material includes indications for surgery, proper care of the surgical wound, possible unwanted complications, their prevention and their symptoms. Furthermore, the patient will find possible aids to improve the application of nasal solutions. Four patients in the ENT, Head and Neck Surgery Department were educated. Results: Four patients were educated in the department of ENT, head and neck surgery. Using an intake questionnaire and interview with individual patients, nursing diagnoses were made and an educational process was designed. Patients were most ignorant about postoperative wound care and complications. Patients reported that they used information from the booklet developed during their hospitalization. All patients took an active approach to education and agreed that they found the education beneficial and enriched their existing knowledge. Conclusion: The pilot education results showed that careful patient education during FESS is important for postoperative care. For practice, I would recommend setting aside enough time and quiet space for patient education.
