Abstract:

Karcinom pankreatu je považován vzhledem ke své infaustní prognóze, pozdnímu záchytu a omezeným možnostem léčby a prevence za jedno z nejzávažnějších onkologických onemocnění moderní civilizace. Tyto skutečnosti jsou ovlivněny zejména multifaktoriální etiopatogenezí onemocnění. Dosud publikované studie identifikovaly ve svých výsledcích pouze dvě oblasti faktorů, které přispívají ke zvýšenému vzniku karcinomu pankreatu. Jednoznačně je prokázán negativní vliv genetických faktorů a vliv kouření. Vysoká míra incidence a mortality populace ČR nás řadí na přední pozice v celosvětovém i evropském měřítku. Cílem této práce byl popis epidemiologické situace v ČR ve vztahu k ostatním zemím světa a identifikace možných rizikových faktorů z dosud publikovaných studií, které by mohly sloužit k tvorbě preventivních postupů. Praktická část se zabývá analýzou faktorů, které byly obsahem dotazníkového šetření grantové studie ?Multicenter Case-Control Study of Pancreatic Cancer?. V této práci jsou analyzována data získaná od respondentů žijících na území Moravskoslezského kraje. Těchto respondentů bylo celkově 120, 60 případů a 60 kontrol. U uvedených osob byly prostřednictvím dotazníku analyzovány oblasti životního stylu, zaměstnání a jeho rizik ve vztahu k poruchám zdraví. Jednotlivé otázky byly vyhodnocovány pomocí popisné statistiky, pomocí OR a 95% IS a pomocí t-testu na hladině významnosti 5%. Výsledky šetření ukázaly, že nynější nebo předchozí výskyt žlučových kamenů v osobní anamnéze by mohl působit jako rizikový faktor karcinomu pankreatu (p=0,01; OR=3,95; 95% IS 1,44-10,84). Abstract:

Pancreatic cancer is considered to be due to their life limiting prognosis, late detection and limited treatment options and prevention as one of the most serious oncological disease of modern civilization. These facts are mainly influenced by a multifactorial disease aethiopatogenesis. So far, published studies identified in its results, only two of the factors that contribute to increased cancer of the pancreas. The negative influence of genetic factors and the influence of smoking are clearly demonstrated. High incidence and mortality rate of Czech population puts us on the leading position in global and European scale. The aim of this work was the description of the epidemiological situation in the Czech Republic in relation to other countries in the word and identifying potential risk factor from published studies that could serve to create prevention strategies. The practical part deals with the analysis of factors that were the contents of the grant survey study ?Multicenter Case-Control Study of Pancreatic Cancer?. This work analyzed data obtained from respondents living in Moravian-Silesian Region. These respondents were generally 120, 60 cases and 60 controls. For these individuals were analyzed by means of a questionnaire on lifestyle, occupation, and its risk on relation to health disorders. Individual question were evaluated using descriptive statistics, using OR and 95% CI and t-test at a significance level of 5%. Survey results showed that current or previous occurrence of gallstones in the personal history could be a risk factor for pancreatic cancer (p=0,01; OR=3,95; 95% CI 1,44 10,84). Other outputs from the practical part of the thesis are presented in the discussion.
