Changes of foot load during pregnancy and postpartum

Abstract:

Diplomová práce sleduje změny rozložení plantárních tlaků a kontaktní plochy ve třech částech chodidla u primigravidních žen během těhotenství a po porodu. V návaznosti na změny těchto parametrů byla sledována případná souvislost s bolestí nohou, projevující se pravděpodobně jako důsledek redistribuovaných tlaků a zvětšení kontaktní plochy chodidla. Posledním sledovaným parametrem byla měnící se laxicita vaziva a její případná korelace s výše uvedenými hodnotami. Výzkumu se zúčastnilo 10 žen. Každá žena byla měřena v pěti termínech. První měření probíhalo do 15. týdne těhotenství. Další ve 23., 30. a 36. týdnu těhotenství. Poslední měření se uskutečnilo v rozmezí 6. - 10. týdnů po porodu. Měření probíhalo na plantografické plošině Emed?-xl firmy Novel. Součástí každého měření bylo vyšetření hypermobility dle Beightona a Horana a hodnocení bolesti nohou pomocí krátké formy Mc-Gillova dotazníku. Plantární tlak i kontaktní plocha chodidla se v průběhu těhotenství postupně zvyšuje ve všech částech chodidla. Po porodu se hodnoty plantárního tlaku v předonoží a zánoží vrací mírně pod původní hodnoty. Ve středonoží přetrvávají zvýšené. Kontaktní plocha chodidla se po porodu výrazně zvětšuje ve všech částech chodidla. Nejvýznamnější změny pozorujeme ve středonoží. Signifikantní rozdíly jsou patrné i mezi jednotlivými měřeními. Bolest nohou koreluje s plantárním tlakem.

Abstract:

This diploma thesis observe changes in the distribution of plantar pressures and changes in the contact area in three parts of the feet of primigravid women during pregnancy and postpartum. In connection with changes in these parameters, a possible association with foot pain was observed, probably resulting from redistributed pressures and increased foot contact area. The last observed parameter was the changing laxicity of ligament and its possible correlation with the values mentioned above. Ten women participated in the research. Each woman was measured in five terms. The first measurement was performed until the 15th week of pregnancy. Following measurements were performed in the 23rd, 30th and 36th weeks of pregnancy. The last measurement was taken 6-10 weeks postpartum. The measurements were carried out on the plantographic platform Emed?-xl by Novel. Each measurement included weighing, measurements on the Emed?-xl platform in a dynamic mode, a Beighton and Horan hypermobility examination and evaluating leg pain using a short form of the Mc-Gill questionnaire. Foot plantar pressure and contact area during pregnancy gradually increase in all parts of the foot. The values of plantar pressure in the foretoot and rearfoot slightly below the origin value postpartum. In midfoot the increase persist. The contact area is significantly enlarge in all parts of foot after pregnancy. The biggest changes we observe in midfoot. Significant differences are also evident between the different measurement. Leg pain is correlated with plantar pressure.
