Physiotherapy after total hip arthroplasty

Abstract:

ABSTRAKT Úvod: Tato bakalářská práce se zabývá tématem fyzioterapie po totální endoprotéze kyčelního kloubu a obsahuje shrnutí teoretických poznatků o anatomii a kineziologii kyčelního kloubu a bederní páteře, dále pojednává o léčebné metodě po totální endoprotéze kyčelního kloubu u konkrétního pacienta. Cíl práce: Cílem této bakalářské práce je přispět k řešení problematiky fyzioterapie po totální endoprotéze kyčelního kloubu. Metodika: V bakalářské práci jsem volila metody nejvhodnější terapie pro pacienta po totální endoprotéze kyčelního kloubu. Pacient byl hospitalizován v FN Bulovka po dobu 4 týdnů. Při vstupním kineziologickém rozboru jsem odebrala anamnézu, vyšetřila držení, konfiguraci a trofiku, dále rozsah pohybu v kyčelním kloubu, svalový tonus, a provedla vyšetření zaměřené na svalovou sílu dle prof. Jandy a to především na stabilizátory kyčelního kloubu. Vyšetřila jsem pohybový stereotyp a stereotyp chůze o dvou francouzských holích. Následně jsem na základě vstupního vyšetření zvolila ideální krátkodobý a dlouhodobý plán fyzioterapie. Mezi fyzioterapeutické metody jsem zařadila cévní a dechovou gymnastiku, pasivní a aktivní pohyby, míčková facilitace, dále techniky měkkých tkání, postizometrická relaxace, rytmická stabilizace, akrální koaktivační terapie?. Pomůcky jsem využila především Theraband, overball, a prvky z Brüggerova konceptu. Výsledky: Na základě výstupního kineziologického rozboru bylo vidět značné zlepšení. Došlo ke zvětšení rozsahu pohybu v kyčelním kloubu především do flexe 90°, extenze (15°), abdukce (30°). Pacient byl bez otoku a bolesti. Zlepšila se svalová síla levé dolní končetiny, hlavně v oblasti gluteálních svalů, a to v průměru o 2 stupně. Došlo ke zlepšení stereotypu chůze. Podařilo se navodit lepší zapojení svalů při pohybu. Závěry: Včasná a cílená fyzioterapeutická intervence přispívá k lepšímu a rychlejšímu návratu do běžných činností a zlepšuje tak kvalitu života pacienta.

Abstract:

SUMMARY Introduction: This bachelor thesis deals with a physical therapy after total hip arthroplasty. It summaries theoretical knowledge about the anatomy and kinesiology of the hip joint and lumbar spine and also describes different methods of physical therapy which can be used in this issue. The main part of the thesis describes the physical therapy intervention in patient with total hip arthroplasty. Aim of the thesis: The aim of this bachelor thesis is to gather knowledge about total hip arthroplasty and to draw up a physical therapy intervention on the basis of the kinesiological analysis of the patient. Methodology: The patient was hospitalized in Bulovka Hospital for 4 weeks. At first I did the initial kinesiological analysis. It consisted of a medical history of the patient and of a visual and palpation examination. I also looked into the range of motion of the joints and muscle strength. After that I drew up the short-term and long-term physical therapy intervention. The short-term intervention included vein therapy exercises and breathing gymnastics, passive and active movements, ball facilitation, as well as soft tissue techniques, post isometric relaxation, rhythmic stabilisation, acral coactivation therapy?. The tools I used were mainly therabands, overballs, and elements from the Brügger concept. Results: Comparison of the initial and output kinesiological analysis showed significant improvement in the patient. The most important is increase of the range of motion of the hip joint (flexion increased by 90°, extension by 15°, abduction by 30°). The patient was free of swelling and pain. Average increase in the muscle strengh of the lower limb was two grade. The biggest improvement was in the gluteal muscles. There was an improvement in gait stereotype. It was possible to induce better muscle involvement during movement. Conclusions: The physical therapy intervention was successful. The patient is doing well on his way back to normal life and he is pain free now. I extended my knowledge in this issue due to this bachelor thesis.
