Problematika kochleární implantace u dospělých osob se sluchovým postižením

Abstract:

Začátek absolventské práce, přesněji teoretická část, odborně popisuje sluchové ústrojí. Vzhledem k tématu kochleárního implantátu jsem jej zaměřila hlavně na tu část sluchového ústrojí, která je s kochleárním implantátem úzce spojena. Dále jsem v krátkosti popsala klasifikaci sluchového postižení, na které navazuje kapitola o zvuku jeho rozhraní, na kterém vnímáme nejen zvuky z okolí, ale také řeč. V poslední teoretické části své práce se zaměřuji na kochleární implantát, jeho podrobný technický popis a využití. Kromě toho rozeberu podmínky, které musí dospělá osoba se sluchovým postižením splnit, aby mohla být zapsána na seznam uchazečů o kochleární implantát. Také popíši následnou rehabilitaci, která je neoddělitelným, náročným a velice důležitým procesem, který teprve rozhodne, jaký přínos kochleární implantát bude mít. Zahrnuji do této oblasti nejen přínos, ale také rizika, která se např. s operací samotnou pojí. Do druhé části absolventské práce patří rozhovory se dvěma osobami se sluchovým postižením. První osoba s implantátem a druhá osoba, která se rozhoduje pro implantaci. Zajímají mne otázky o jejich potřebách, proč kochleární implantát chtějí (chtěli). Jestli byli seznámeni se všemi problematickými body, které implantace přináší. Co je (bylo) pro ně důvod přemýšlet a váhat. Z čeho konkrétně plynou obavy a co by jim pomohlo tuto obavu zmírnit nebo úplně odstranit. Cílem těchto rozhovorů pro mě bylo zjistit obavy, které se k tak náročné operaci přirozeně vztahují. Nejen obavy, ale také zkušenosti, které by poskytla osoba již implantovaná. Mít následně možnost tyto zkušenosti sdílet pro osoby se sluchovým postižením, které váhají podobným způsobem. Mám za to, že sdílení je efektivní způsob osvěty, který může zmírnit dopad náročného průběhu celého procesu, který k implantaci patří.

Abstract:

The first chapter of the theoretical part technically describes human's auditory system. Due to the topic of a cochlear implant I aimed mainly on the part of the auditory system, which is closely associated with the implant. In the following chapter I briefly described classification of hearing impairment which is followed up by chapter about the sound of its interfaces, in which we not only percieve the sounds of the surroundings but also speech. Last chpater in the theoretical part is focused on the cochlear implant, its detailed technical description and its usage. Furthermore, I analysed the conditions which needs to be meet by a person with hearing disabilities in order to be added to the list of candidates for cochlear implant. Then I describe the subsequent rehabilitation - inseparable, difficult and very important process that decides what benefit the cochlear implant will have. I include in this chapter not only benefits but also risks associated with such an operations. The second part of my graduate work includes interviews with two persons with hearing disabilities. First person has had the implant already and second one is deciding about implantation. I am asking them about their needs, why they want (or wanted) cochlear implant. Also I am curious if they were introduced to all the problematic aspects of implantation. What is (or was) for them a reason to think and hesitate about. What are the sources of fear and what could help them to mitigate this fear or totally get rid of it.. The purpose of these interviews was to determine which concerns are naturally related to such a challenging operation. Not only concerns but also experience of the person who has implant already. Sharing this experience could be subsequently beneficial for people with hearing disabilities, who are personaly considering implantation. I believe, sharing is an effective way to spread enlightenment which may mitigate the impact of the complicated implantation process.
