The use of HPV testing in cervical cancer screening

Abstract:

Karcinom děložního hrdla je celosvětově třetí nejčastější malignitou ženského genitálu s vysokou incidencí a mortalitou. Nejvýznamnějším etiologickým faktorem podílejícím se na vzniku tohoto onemocnění je infekce vysoce rizikovým typem lidského papilomaviru (HR HPV), který se přenáší pohlavním stykem. V případě včasného záchytu dysplastických či počínajících nádorových změn může pacientka podstoupit nemutilující, přesto dostatečně radikální a sanativní léčbu. V souboru 293 pacientek vyšetřených cytologicky a následně i molekulárně-geneticky byla hodnocena exprese HR HPV u jednotlivých cytologických nálezů, nejčetnější subtypy HPV a statisticky byla hodnocena závislost HR HPV na cytologické diagnóze. HPV testace je využitelná v případech s nejednoznačnou cytologickou diagnózou (ASC-US, ASC-H) a u diskrepantních cytologických a kolposkopických nálezů. Molekulárně-genetické vyšetření se navíc jeví jako velmi vhodné u cytologické diagnózy AGC-NOS. V budoucnu lze očekávat zařazení HPV testace mezi screeningové metody pro detekci karcinomu děložního hrdla.

Abstract:

Cervical cancer is the third most common female genital tract malignancy worldwide, with high incidence and mortality. An infection with high-risk-type human papillomavirus (HR HPV), which is sexually transmitted disease, is the most significant etiologic factor contributing to the development of this disease.. In the case of an early detection of dysplastic or incipient neoplastic changes, a patient can undergo non–mutilating, yet sufficiently radical and sanative treatment. An expression of HR HPV was examined in individual cytological findings, the most frequent HPV subtypes were evaluated and a correlation between HR HPV and cytology diagnosis was statistically assessed in a group of 293 patients who were cytologically examined and molecular-genetically tested. HPV testing is usable in cases with unclear cytology diagnosis (ASC-US, ASC-H) and for inconsistent cytological and colposcopic findings. Moreover, molecular-genetic testing seems to be very appropriate for cytology diagnosis AGC-NOS. In the future, an integration of HPV testing among screening methods for cervical cancer detection can be expected.
