The psychotherapy of depression in Czech Republic: The comparation of literature and practice

Abstract:

Cílem práce je poskytnout systematický přehled odborné literatury týkající se psychoterapie deprese a porovnat terapii deprese, jak je popisována v odborné literatuře a jak je u nás prováděna v praxi. Pozornost je věnována možným shodám a rozdílům v přístupu k terapii deprese. Hlavní náplní teoretické části je popis jednotlivých psychoterapeutických směrů, které se depresi věnují (kognitivně-behaviorální terapie, interpersonální terapie, psychodynamická, systemická terapie, humanistická psychoterapie, gestalt terapie atd.), respektive jejich přístupů k léčbě deprese. Popsány jsou také jiné způsoby léčby; pozornost je věnována jejich kombinaci s psychoterapií (zejména kombinaci farmakoterapie s psychoterapií). Diskutovány jsou také studie zkoumající účinnost jednotlivých terapeutických přístupů. Empirickou část tvoří obsahová analýza běžně dostupné česky psané odborné literatury, která se věnuje psychoterapii deprese u dospělých pacientů. Předmětem analýzy jsou odborné knihy, kapitoly v knihách a články z odborných periodik (např. Československá psychologie, Psychiatrie pro praxi, Psychiatria, psychoterapia, psychosomatika, Psychoterapie, Konfrontace atd.). Data jsou zpracována pomocí popisné statistiky. V další fázi výzkumu jsou výsledky analýzy porovnávány s informacemi získanými pomocí dotazníku zaměřeného na praktikování psychoterapie deprese v psychiatrických léčebnách a na psychiatrických odděleních nemocnic v České republice. Nejčastěji doporučovaným psychoterapeutickým přístupem k depresi je kognitivně-behaviorální terapie. Dalším pak interpersonální a rodinná terapie. V praxi je k léčbě deprese nejčastěji využívána kognitivně-behaviorální terapie a psychodynamická terapie. Ostatní terapeutické přístupy jsou využívány spíše okrajově. Výsledky jsou interpretovány a uvedeny do souvislostí se zjištěními z předchozích studií. Na závěr jsou diskutována omezení výzkumu a navrhnuty další výzkumné kroky.

Abstract:

The aim of this study is to create a systematic review of the expert literature concerning the psychotherapy of depression and to compare it with practical psychotherapy of depression. I focus on the consensus and differences concerning the psychotherapeutic approach to depression. The first part consists of the description of the approaches (cognitive-bevioral therapy, interpersonal therapy, psychodynamic therapy, systemic therapy, humanistic therapy, gestalt therapy etc.). Others forms of treatment are discussed here as well as their combination with psychotherapy (especially the combination of psychotherapy and pharmacotherapy). Moreover, I discuss some studies concerning the effectiveness of each approach. Content analysis was used to analyze czech expert literature, which is dealing with the psychotherapy of depressive disorders among adults. The chapters in periodicals (like Československá psychologie, Psychiatrie pro praxi, Psychiatria, psychoterapia, psychosomatika, Psychoterapie, Konfrontace), books and articles are the objects of content analysis. The descriptive statistics was used to work with the data. The results are compared with the information from psychiatric clinics reached by the questionnaire. Cognitive-behavioral therapy was the most frequently recommended psychotherapeutic approach. The second one is interpersonal therarpy and family therapy. Cognitive-behavioral therapy and psychodynamic therapy are the most often used approaches at psychiatric clinics. Others approaches are used marginally. Finally, the results are interpreted and some suggestions concerning the future research are discussed.
