Treatment of Dysplastic Acetabulum Using Biconical Threaded Cup

Abstract:

Totální náhrada kyčlí v léčbě postdysplastické koxartrózy představuje specifický problém,který spočívá v anatomických změnách acetabula a proximálního femuru,v abnormálním centru rotace kyčle,v diskrepanci délky končetin,v insuficienci pelvifemorálního svalstva a často ve zbytnělém kontrahovaném kloubním pouzdru. Z těchto důvodů je implantace totální náhrady kyčle obtížnější než u jiných koxartróz.V závislosti na stupni dysplazie představuje největší problém řešení defektního acetabula. V odborné literatuře jsou neustále diskutovány nejrůznější operační techniky. Cílem mé disertační práce je srovnání mezi střednědobými zkušenostmi s řešením dysplastického acetabula pomocí bikonické závitořezné jamky u vlastního souboru pacientů a mezi výsledky různých operačních metod uváděných v literatuře.

Abstract:

Total hip arthroplasty in the treatment of postdysplastic coxarthrosis is a rather specific procedure due to certain problems which include: anatomical changes of the acetabulum and proximal femur, abnormal hip rotation centre, leg length discrepancies, insufficiency of pelvifemoral muscles, and often contracted thickened joint capsule. This is the reason why total hip replacement in postdysplastic terrain is more complex than in other types of coxarthrosis. Based on dysplasia degree, the biggest problem is the reconstruction of a defective acetabulum. Various operating techniques are being constantly discussed within scientific literature. The aim of the thesis is to compare intermediate experience with the management of dysplastic acetabulum with a biconical threaded cup in our own group of patients and results achieved when using various other operative techniques as presented in literature.
