Fear of fall in seniors

Abstract:

Cíl: Cílem práce bylo zjistit, jak velký je strach z pádu u geriatrických pacientů, kteří jsou hospitalizováni ve zdravotnickém zařízení a u těch, kteří pobývají v různých zařízeních sociální péče. Dalším cílem bylo zjistit rozdíl mezi oběma skupinami. Metodika: Pro dosažení cílů byla zvolena kvantitativní metoda dotazníkového šetření. Strach z pádu byl zjišťován pomocí české verze standardizovaného dotazníku FES-I. Dále byly použité testy ADL a IADL. Výzkumný soubor byl tvořen 162 respondenty ve věku nad 65 let, kteří byli hospitalizovaní na různých odděleních v Uherskohradišťské nemocnici a. s. nebo pobývali v různých zařízeních sociální péče v okrese Uherské Hradiště. Pro srovnání sledovaných parametrů v závislosti na věku, pohlaví a stavu byl použit Chí-kvadrát test. Závislost mezi sledovanými parametry byla hodnocena Pearsonovým korelačním koeficientem (r). Dále byl použit Spearmanův korelační koeficient (rs) pro vztahy mezi testy FES-I, ADL a IADL. Statistické testy byly hodnoceny na 5% hladině významnosti. Statistické hodnocení bylo prováděno v statistickém programu STATA v. 10 Výsledky: Senioři pobývající v zařízeních sociální péče měli statisticky významně větší strach (měřen FES-I), než senioři hospitalizováni ve zdravotnickém zařízení (p = 0,000). S vyšším věkem narůstá i strach z pádu u seniorů (p = 0,002). Medián strachu (FES-I) dosáhl 47 bodů (28-64 vysoké obavy) a je vyšší u skupiny seniorů pobývajících v zařízeních sociální péče. Senzitivita FES-I testu v souvislosti s pádem v anamnéze je 96 % a specificita 19 %. Větší strach z pádu měli ženy (p = 0,011), senioři, kteří již dříve prodělali pád (p = 0,002). Chí kvadrát testem byly potvrzeny statisticky významné vztahy mezi strachem z pádu u seniorů a ADL testem soběstačnosti seniorů (p = 0,000) a IADL testem instrumentačních denních činností (p = 0,000). Naše studie také potvrdila významný korelační kladný vztah mezi testy ADL a IADL (rs = 0,7381) a silný záporný vztah mezi testy FES-I a IADL (rs = - 0,7799) a testy FES-I a ADL (rs = - 0,6823). Z analýzy vyplynulo, že hodnocení strachu z pádu je závislé na stupni soběstačnosti seniorů. Čím méně jsou senioři soběstační a čím víc jsou závislí na pomoci druhých, tím větší mají strach z pádu. Závěr: Strach z pádu je vyšší u žen, u seniorů, kteří již dříve prodělali pád a stoupá se zvyšujícím se věkem a zvýšenou závislostí seniorů.

Abstract:

Objective: The aim was to find out how big the fear of falling in elderly patients who are hospitalized in a medical facility and those who reside in different equipment social care. Another objective was to determine the difference between the two groups. Methods: To achieve the objectives was made a quantitative questionnaire survey method. Fear of falling was assessed using a standardized questionnaire Czech version of the FES-I. Further were used tests ADL and IADL. The research sample consisted of 162 respondents aged over 65 years who were hospitalized in different wards of the hospital as Uherske Hradiste or resided in a variety equipment of social care in the district of Uherske Hradiste. For comparison of monitored parameters depending on the age, sex and condition was made the chi-square test. The dependence between the monitored parameters was assessed by Pearson correlation coefficient (r). Next was used Spearman's rank correlation coefficient (rs) for the relationship between tests FES-I, ADL and IADL. Statistical tests were evaluated at the 5% significance level. Statistical evaluation was performed in the statistical program STATA v. 10 Results: Seniors residing in social care had statistically significantly greater fear (measured FES-I) than seniors hospitalized in a medical facility (p = 0,000). Fear of falling increases with increasing age (p = 0,002). Median fear (FES-I) reached 47 points (28-64 high concern) and is higher in the group of seniors residing in social care. Sensitivity FES-I test in connection with the collapse of a history of is 96 % and specificity of 19 %. Greater fear of falling had women (p = 0,011) and seniors who have previously suffered a fall (p = 0,002). Chi-square tests were confirmed statistically significant relationship between fear of falling in older adults and seniors ADL self-sufficiency test (p = 0,000) and IADL test Instrumentation daily activities (p = 0,000). Our study also confirmed a significant positive correlation relationship between tests ADL and IADL (rs = 0,7381) and a strong negative relationship between test FES-I and IADL (rs = - 0,7799) and test FES-I and ADL (rs = - 0 ,6823). The analysis showed that the assessment of fear of falling is dependent on the degree of self-sufficiency seniors. The seniors who are less self-sufficient and the more depend on the help of others have greater their fear of falling . Conclusion: Fear of falling is higher among women, older people who have previously experienced a fall and increases with growing age and increased dependency seniors.
