Pain management in a surgical wards

Abstract:

Úvod: Ve všech lékařských oborech došlo za poslední desítky let k obrovským pokrokům. Chirurgie je jedním z oborů, ve kterém se staly za poslední roky převratné změny. V souvislosti s pokrokem v medicíně dochází i k neustálému zvyšování kvality v lůžkové péči, s čímž přímo souvisí i zvyšování kvality managementu bolesti. Cíl: Cílem diplomové práce bylo analyzovat kvalitu managementu bolesti na chirurgických odděleních v KZ a.s. v Litoměřicích. Metodika: Byla použita kvantitativní metoda šetření. Dotazníky byly respondentům předávány osobně v rámci pracovní doby. Soubor tvořily všeobecné sestry, které jsou zaměstnané na jednom z vybraných reprezentativních oddělení (aseptická chirurgie, septická chirurgie či ortopedie). Výsledky dotazníkového šetření byly statisticky vyhodnoceny. Výsledky: Průzkumného šetření se zúčastnilo 60 respondentů, z toho 60 žen a 0 mužů. Nejčastěji dotazník vyplňovali respondenti ve věku 50-59 let. Z výsledků vyplývá, že 96,7 % respondentů vědělo o vypracovaném standardu pro sledování a hodnocení bolesti na svém pracovišti. 16,7% respondentů hodnotilo spolupráci lékařů a všeobecných sester při léčbě bolesti výborně a 50% respondentů velmi dobře. Pro 95 % respondentů byla pozornost lékařů, věnovaná pacientům při projevech bolesti dostatečná. Nejčastějším zdrojem informací o managementu bolesti byli kolegové, a to u 53,3 % respondentů. 95% respondentů bylo spokojeno se způsobem léčby na svém pracovišti a 81,7% respondentů se domnívalo, že má dostatečné informace o managementu bolesti. Závěr: Nespokojenost multidisciplinárního týmu se vzájemnou spoluprací může mít negativní dopad na kvalitu managementu bolesti, která je klíčová při řešení pacientovy bolesti. Cílem každého zdravotnického zařízení by mělo být zajištění multidisciplinárního týmu, utvořeným kvalifikovaným personálem, který spolu navzájem spolupracuje a cíleně se podílí na léčbě pacientovy bolesti.

Abstract:

Introduction: Huge advances have been made in all medical fields over the past decades. Surgery is one of the fields in which revolutionary changes have taken place in recent years. In connection with the progress in medicine, there is also a continuous improvement in the quality of inpatient care, which is directly related to the improvement of the quality of pain management. Aim: The aim of the diplom thesis was to analyze the quality of pain management on surgical departments in KZ a.s. in Litoměřice. Methodology: A quantitative method of investigation was used. The questionnaires were handed over to the respondents in person during working hours. The group consisted of general nurses who are employed in one of the selected representative departments (aseptic surgery, septic surgery or orthopaedics). The results of the questionnaire survey were statistically evaluated. Results: 60 respondents participated in the survey, of which 60 were women and 0 were men. The questionnaire was most frequently completed by respondents aged 50-59 years. The results show that 96.7% of respondents were aware of the developed standard for monitoring and evaluating pain in their workplace. 16.7% of respondents rated the cooperation of doctors and general nurses in the treatment of pain as excellent and 50% of respondents very well. For 95% of respondents, the attention paid by doctors to patients in case of pain was sufficient. The most common source of information about pain management was colleagues, with 53.3% of respondents. 95% of respondents were satisfied with the way they were treated in their workplace and 81.7% of respondents believed that they had sufficient information about pain management. Conclusion:The dissatisfaction of a multidisciplinary team with mutual cooperation can have a negative impact on the quality of pain management, which is key in dealing with the patient's pain. The goal of every medical facility should be to provide a multidisciplinary team consisting of qualified staff who cooperate with each other and participate in the treatment of the patient's pain.
