Coordinated rehabilitation - application on client with cerebral palsy

Abstract:

Cílem této teoreticko-aplikační práce je popsat a analyzovat koncept koordinované rehabilitace u klienta s dětskou mozkovou obrnou. Teoretická část práce je rozdělena do čtyř hlavních kapitol. První kapitola teoretické části práce objasňuje koncept koordinované rehabilitace, popisuje její principy, cíle, možnosti a využití. Druhá kapitola práce nám vysvětluje pojem zdravotní postižení, jedná se o základní definice této složité tématiky. Třetí kapitola teoretické části práce je věnována tématice dětské mozkové obrny a čtvrtá kapitola objasňuje roli sociálního pracovníka v konceptu koordinované rehabilitace. V aplikační části práce navazuji na teoretické poznatky, které byly získány na základě odborné literatury. Cílem aplikační části práce je analyzovat koncept koordinované rehabilitace skrze aplikaci u klienta s dětskou mozkovou obrnou. Data pro vytvoření případové kazuistiky byla shromažďována prostřednictvím sekundárních zdrojů. Aplikační část práce se soustřeďuje na metodologické ukotvení a samotnou případovou kazuistiku. Případová kazuistika obsahuje anamnézu klienta, průběh, výstup a návrhy možného řešení nedostatků. K případové kazuistice jsem vybrala dětského klienta, který využívá denního pobytu v Dětském rehabilitačním stacionáři v Ostravě - Porubě. Jedná se o detašované pracoviště Městské nemocnice Ostravy.

Abstract:

The aim of this theoretical work with its practical application is to describe and analyze the concept of coordinated rehabilitation of clients with cerebral palsy. The theoretical part is divided into four main chapters. The first chapter explains the concept of coordinated rehabilitation; it describes its principles, goals, possibilities, and applications. The second chapter explains the concept of disability; laying down the basic definitions of this complex topic. The third chapter of the theoretical part is devoted to the topic of cerebral palsy and the fourth chapter clarifies the role of social workers in the concept of coordinated rehabilitation. In the practical part I follow up on the theoretical knowledge gained from professional literature. The aim of the practical part is to analyze the concept of coordinated rehabilitation through its application on a client with cerebral palsy. The data presented in the case study was collected using secondary sources. The application part focuses on methodological background and the actual case study. The case study contains a medical history of the client, the process, outcome, and suggestions on how to potentially solve any shortcomings. For the case study I chose a child client, who attends daily the Children's rehabilitation ward in Ostrava-Poruba, which is an external branch of the Ostrava Municipal Hospital.
