Abstract:

Bolest je nedílnou součástí každého operačního výkonu. Bolestivé projevy jsou hlavním indikátorem a součástí řady onemocnění. Včasně a kvalitně diagnostikovaná bolest a následná dostatečná léčba jsou základem každého úspěšného operačního výkonu a pooperačního období. Nedostatečná pooperační analgezie může mít nežádoucí účinky na oběhový, dýchací, trávicí a imunitní systém. V krajním případě může přejít do chronicity a následně zhoršit kvalitu života. Diplomová práce je zaměřena na problematiku perioperační a pooperační bolesti u pacientů po operaci žlučníku, tříselných kýl, ledvin, prostaty a dělohy. Cílem diplomové práce je analýza hodnocení bolesti na zvolených pracovištích a průběh analgetizace pacientů v perioperační péči. Dále budou srovnány rozdíly v intenzitě bolesti u různých druhů operačních výkonů. Po analýze dat získaných z anonymních dotazníků lze konstatovat, že intenzita bolesti se liší v závislosti na operaci. Největší bolest byla uváděna po operaci ledvin, prostaty a dělohy, naopak nejmenší pooperační bolest odpovídá operaci kýly. Intenzita bolesti je nejsilnější mezi 2.-12. hodinou po operaci. Rozdíl v intenzitě bolesti mezi laparotomicky a laparoskopicky provedenou operací nebyl v cíleném dotazníkovém šetření potvrzen, obdobně jako v případě epidurální anestézie.

Abstract:

Pain is an unnecessary part of each surgery. Pain as a symptom is one of the main indicators and a part of most illnesses. Fast and precise diagnosis of pain is fundamental for each and every successful surgical intervention and recuperation. Insufficient analgesia can cause severe problems with cardiovascular, respiratory, digestive and immune systems. In extreme cases pain can progress to chronicity and deteriorate quality of life. This diploma thesis is focused on perioperative and post-operative pain after gallbladder, gastric, inguinal hernia, kidney, prostate and uterus surgery. The objective of this diploma thesis is analysis of pain evaluation in perioperative care at selected workplaces and process of patients analgesia in perioperative care. Furthermore, there will be comparison of pain intensity in several surgical interventions. After the data analysis of anonymous questionnaires, it is safe to say that pain intensity varies depending on the surgical intervention. The highest pain intensity was reported for kidney, prostate and uterus surgery. In the opposite, the lowest post-operative pain was associated with inguinal hernia surgery. Intensity of the pain is strongest between two to twelve hours after the intervention. Difference of pain intensity between laparotomic and laparoscopic surgery was not confirmed in the study, as well as epidural anesthesia.
