Abstract:

Předkládaná práce shrnuje výsledky zkoumání růstu a vývoje dětí Evropské longitudinální studie těhotenství a dětství – ELSPAC. Hlavní náplní práce je představení metod, které jsou běžně využívány k hodnocení růstu a vývoje a hledání té z nich, která je nejvhodnější pro hodnocení longitudinálních dat studie ELSPAC. Tato data jsou unikátní svým rozsahem, zaměřením i možností mezinárodního srovnávání. Přímo se nabízejí k hodnocení faktorů ovlivňujících zdraví a vývoj jedince od narození až do dospělosti. Při hledání způsobů nejefektivnějšího zhodnocení dat jsme se čím dál více klonili k individuálnímu přístupu v růstové analýze. Díky autorovi metody nazvané Dynamický fenotyp, který nám nabídl možnost testování tohoto individuálně založeného modelového přístupu, jsme získali způsob hodnocení ideálně odpovídající naší široké databázi. Dynamický fenotyp je unikátní tím, že vychází z fyziologické podstaty růstu, zaměřuje se na biologického jedince a modeluje jeho růst od okamžiku vzniku jeho genomu splynutím vajíčka a spermie. Jak vyplývá z názvu metody, jedná se o modelování fenotypového projevu růstového programu v růstu konkrétního jedince. Každá z komponent modelové růstové křivky je řízena matematickou funkcí (Gompertzovou a dvěma logistickými funkcemi) a je charakterizována třemi biologicky interpretovatelnými parametry – počáteční a limitní hodnotou a hodnotou maximálního přírůstku v inflexním bodu křivky. Funkčnost metody byla ověřena na datech Brněnské longitudinální studie doc. Bouchalové a studie ELSPAC. Byly vytvořeny pravé růstové křivky z longitudinálních růstových řad účastníků studie ELSPAC a na základě těchto křivek byly provedeny rozšiřující analýzy.

Abstract:

The presented dissertation thesis summarizes the results of growth and development research in children participating in the European Longitudinal Study of Pregnancy and Childhood – ELSPAC. The general objective of the dissertation is an introduction to the methods commonly used to evaluate growth and development and finding the one most suitable for analyzing longitudinal data from the ELSPAC study. These data are unique for their range, focus and the readiness for international comparison. The data are ideal for evaluating the factors which influence health and development of an individual from birth until adulthood. Along the path to finding the most effective way of evaluating the available growth data the individual approach to growth analysis was becoming gradually more and more preferred. Thanks to the author of the Dynamic Phenotype method who offered the possibility to test his individual-based model approach on longitudinal data, a manner of growth data evaluation ideal for our wide database was acquired. The Dynamic Phenotype method is unique for its physiological approach to growth modelling, for its concentration on the biological individual and for modelling its growth from the moment of creation of its genome when the egg and sperm cell merge. As the name of the method suggests, Dynamic Phenotype models the phenotype expression of the growth program in the growth of the particular individual. Each of the model growth curve components is controlled by a mathematical function (one Gompertz and two logistic functions) and is characterized by three biologically interpretable parameters – the starting and limiting (asymptotic) values and by the value of maximum gain in the inflection point of the curve. The functionality of the method was verified on the data from the Brno longitudinal study and on the ELSPAC study data. Genuine growth curves were created from longitudinal growth successions of ELSPAC participants and extensive analyses were carried out based on these curves.
