Hodnocení kvality života a psychosociálních aspektů nemoci u pacientů se srdečním selháním

An Assessment of Quality of Life and Psychosocial Aspects of Illness of Patients with Heart Failure

Abstract:

Úvod: Srdeční selhání je spojeno s řadou příznaků, které mají potenciál výrazně ovlivňovat život pacienta. Cíl: Cílem výzkumu bylo vyhodnotit kvalitu života a vybrané psychosociální aspekty nemoci (deprese, vnímání nemoci a sociální podporu) u pacientů se srdečním selháním. Metodika: Kvalita života byla hodnocena pomocí specifického nástroje Minnesota Living with Heart Failure Questionnaire, deprese prostřednictvím Patient Health Questionnaire-9, vnímání nemoci pomocí nástoje Brief Illness Perception Questionnaire, a sociální podpora pomocí ENRICHD Social Support Inventory. Data byla získávána na pracovištích kardiovaskulárního oddělení vybraného zdravotnického zařízení fakultního typu od července 2020 do listopadu 2021. Pro analýzu dat byl použit program Stata verze 14, statistické testy byly hodnoceny na hladině významnosti 5 %. Výsledky a diskuze: Výzkumný soubor zahrnoval 173 pacientů s průměrným věkem 71,51 let (SD = 8,62; min. 50; max. 92). Muži tvořili 56,07 % výzkumného souboru. Průměrné skóre kvality života bylo 44,86 bodu (SD = 19,31). Statisticky významné vztahy byly zjištěny mezi skóre kvality života ve fyzické doméně a věkem (p = 0,0224), dále mezi skóre kvality života ve fyzické doméně a NYHA klasifikací (p = 0,0007) a mezi celkovým skóre kvality života a NYHA klasifikací (p = 0,0028). Byl zjištěn také statisticky významný vztah kvality života a soběstačnosti, a to jak v rámci celkového skóre kvality života (p = 0,0002), tak v rámci doménového skóre. Dále byl zjištěn také statisticky významný vztah mezi kvalitou života a kategoriemi deprese (p = 0,0001) jak v rámci celkového skóre, tak ve skóre jednotlivých domén. Statisticky významný vztah existoval také mezi kvalitou života a jednotlivými kategoriemi subjektivního hodnocení zdravotního stavu (p = 0,0001) jak v rámci celkového skóre, tak skóre v jednotlivých doménách. Výzkum dále odhalil souvislost mezi kvalitou života a vnímáním nemoci (r = 0,4113; p < 0,05). Statisticky významnými prediktory celkového skóre kvality života byly deprese, vnímání nemoci, soběstačnost a subjektivní hodnocení zdravotního stavu. Závěr: Horší kvalitu života udávali pacienti se závažnějším srdečním selháním dle NYHA klasifikace, pacienti se sníženou soběstačností, a pacienti s depresí. Ti také hůře subjektivně hodnotili svůj zdravotní stav a hůře vnímali své onemocnění. Přítomnost deprese, horší vnímání nemoci, snížená soběstačnost a horší subjektivně hodnocený zdravotní stav byly vyhodnoceny jako prediktory špatné kvality života. Srdeční selhání je tedy komplexní problém, jehož řešení vyžaduje holistický přístup.

Abstract:

Introduction: Heart failure is associated with a number of symptoms that could potentially affect the patient's life. Aim: The aim of the research was to evaluate the quality of life and selected psychosocial aspects of the disease (depression, illness perception and social support) of the patients with heart failure. Methods: Quality of life was assessed by using the Minnesota Living with Heart Failure Questionnaire, depression by the Patient Health Questionnaire-9, illness perception by the Brief Illness Perception Questionnaire, and social support by the ENRICHD Social Support Inventory. Data were collected at the cardiovascular ward of a selected faculty-type medical facility from July 2020 to November 2021. The Stata version 14 program was used for data analysis, statistical significance level was set on 5%. Results and discussion: The sample included 173 patients with an average age of 71.51 years (SD = 8.62; min. 50; max. 92), 56.07% of the sample were men. The mean quality of life score was 44.86 points (SD = 19.31). Statistically significant relationships were found between the quality of life score in the physical domain and age (p = 0.0224), also between the quality of life score in the physical domain and NYHA classification (p = 0.0007) and between the overall quality of life score and NYHA classification (p = 0.0028). A statistically significant relationship between quality of life and self-sufficiency was also found, both within the overall quality of life score (p = 0.0002) and within the domains score. Furthermore, a statistically significant relationship between quality of life and categories of depression (p = 0.0001) was also found both within the overall score and in the scores of individual domains. There was also a statistically significant relationship between quality of life and individual categories of subjective assessment of state of the health (p = 0.0001) both within the overall score and scores in individual domains. The link between quality of life and illness perception (r = 0.4113; p < 0.05) was found. Statistically significant predictors of the overall quality of life score were depression, illness perception, self-sufficiency and subjective assessment of state of the health. Conclusion: A worse quality of life was reported by patients with more severe heart failure according to the NYHA classification, patients with reduced self-sufficiency, and patients with depression. They also had a worse subjective assessment of their state of health and a worse illness perception. The presence of depression, worse illness perception, reduced self-sufficiency and worse subjective assessment of state of the health were evaluated as predictors of poor quality of life. Therefore, heart failure is a complex problem, the solution of which requires a holistic approach.
