Specifics nursing care of the patient with yew intoxication

Abstract:

Tis červený (Taxus baccata) je jehličnan, který se vyskytuje na zahradách a volně v parcích. Alkaloidy taxiny jsou obsaženy v semenu, jehličí a kůře. Pouze červený míšek, obalující semeno není jedovatý. Letální dávka je odhadována na 50 gramů odvaru z jehličí či jeho požití. Mladší část rostliny obsahuje silnější toxické látky, než starší části rostliny. Pacienti, kteří užili smrtelnou dávku, tak většinou umírají v kardiogenním šoku při prodloužené resuscitační péči, jako reverzibilní příčině srdeční zástavy. Proti této toxické rostlině neexistuje specifické antidoum, ale předpokládá se, že se dá tzv. systolická fáze překonat pomocí podpory extrakorporální membránové oxygenace. O doporučených postupech se píše pouze na základě zkušeností v kazuistikách a není ověřené, zda použité metody byly opravdu účinné, nebo jestli pacient požil netoxickou dávku, či starší část rostliny, která nevykazuje tak vysokou toxicitu, jako mladší část rostliny.

Abstract:

Yew (Taxus baccata) is a conifer, which occurs in gardens and loosely in parks. Alkaloids taxin are contained in seeds, needles and bark. Only the red sprouts that cover the seed are not poisonous. The lethal dose is estimated for 50 grams of the needle sauce or its ingestion. The younger part of the plant contains stronger toxic substances than the older part of the plant. Patients who have taken a lethal dose usually die in cardiogenic shock in prolonged resuscitation as a reversible cause of cardiac arrest. There is no specific antidote against this toxic plant, but it is assumed that the so-called systolic phase can be overcome with the help of Extracorporeal Membrane Oxygenation. The recommended procedures are written only on the basis of experience in case studies and it is not verified whether the methods used were really effective or whether the patient took the non-toxic dose or the older part of the plant that does not show as high toxicity as the younger part of the plant.
