Epidemiology and prevention of cervical cancer

Abstract:

V České republice je karcinom děložního čípku ročně diagnostikován téměř u 1000 žen, přičemž okolo 400 žen na toto onemocnění každoročně umírá. Incidence se v České republice pohybuje dlouhodobě okolo 20/100 000 žen. V porovnání s vyspělými zeměmi Evropy nejsou tato čísla nijak příznivá. Etiologickým agens, který hraje jednu z nejvýznamnějších rolí ve vzniku tohoto onemocnění je infekce lidským papilomavirem (Human papillomavirus ? HPV). Infekce HPV je nejčastějším sexuálně přenosným onemocněním. HPV rozdělujeme podle jejich onkogenního potenciálu na nízko a vysoce rizikové typy. Celkem ze 14 vysoce rizikových typů jsou typy HPV 16 a HPV 18 považovány za nejrizikovější. Perzistentní infekce těmito typy HPV vede k iniciaci karcinogeneze. V teoretické části práce je největší část věnovaná kapitole týkající se epidemiologie karcinomu děložního čípku a prevence. V práci je podrobně rozebrána problematika primární a sekundární prevence. Dále je v teoretické části pozornost zaměřená na popis anatomie děložního čípku, prekanceróz, maligních nádorů děložního čípku, popis rizikových faktorů a zvlášť je také věnovaná kapitola týkající se HPV. Praktická část je zaměřená na sekundární prevenci a to zejména na možnosti detekce HPV. Cílem práce bylo porovnat dvě screeningové metody používané k detekci HPV a srovnat je s výsledky cytologického vyšetření. V CGB laboratoři v Ostravě bylo testováno 100 vzorků od pacientek dvěma systémy. Prvním je systém Abbott, který detekuje přítomnost DNA 14 vysoce rizikových typů HPV a druhým je systém Aptima, který detekuje přítomnost E6/E7 virových onkoproteinů messengerové RNA (mRNA) 14 vysoce rizikových typů HPV. V práci je podrobně popsán postup manuální izolace, izolace poloautomatickým systémem Abbott a postup automatickým systémem Aptima. Výsledkem práce bylo, že rozdíl mezi testovanými metodami není statisticky významný a obě metody jsou tedy zaměnitelné. Výsledky se lišily pouze ve třech případech, které jsou podrobně rozebrány v diskuzi. Dále jsou v práci statisticky zpracovány údaje o věkovém rozložení zkoumaného souboru žen, výsledcích cytologického vyšetření a výsledcích HPV testování u ASC-US, LSIL a HSIL výsledků cytologického vyšetření.

Abstract:

In the Czech Republic every year, almost 1 000 women are diagnosed with cervical cancer, 400 die because of this disease every year. Incidence is at 20/100 000 women. These figures are not favourable when compared to other European countries. Etiologic agent playing the most significant role in development of this disease is the human papillomavirus infection (HPV). HPV infection is in most cases sexually transmitted. HPV is classified based on its oncogenic potential into low- and high-risk types. Out of a total number of 14 high-risk types, HPV 16 and HPV 18 are considered to be posing the highest risk. Persistent infection with these types of HPV leads to carcinogenesis initiation. The theoretical part of my thesis is dedicated to cervical cancer epidemiology and prevention. The thesis deals in detail with the primary and secondary prevention. Furthermore, the theoretical part focuses on the anatomical description of the cervix, precancerous cells, malign tumours of the cervix, description of risk factors and a whole chapter is dedicated to the HPV. The practical part focuses on the secondary prevention, especially to the options of HPV detection. The aim of the thesis was to compare two screening methods used to HPV detection. In a CGB laboratory in Ostrava, I tested 100 samples collected from female patients. This testing was done using two systems. The first one is the Abbott system that detects presence of DNA of 14 high-risk types of HPVs. The second one is the Aptima system that detects presence of E6/E7 viral oncoproteins of the messenger RNA (mRNA) of 14 of the high-risk HPVs. The thesis contains a detailed description of manual isolation, isolation using the semi-automatic Abbott system and isolation using the automatic Aptima system. The results show that the statistical differences between the two tested methods are not significant and both the methods are interchangeable. The results differed only in three cases that are analysed in detail in the discussion. Furthermore, the thesis contains statistically processed data regarding the age spectrum of the tested females, results of cytological check-up and results of HPV tests with the ASC-US, LSIL and HSIL results of the cytological check-ups.
