Predictive biomarkers of the development of febrile seizures

Abstract:

Febrilní křeče (FK) představují nejčastější typ záchvatovitého onemocnění v dětském věku, Typicky se vyskytují u dětí mezi 6. měsícem a 5. rokem věku s maximem mezi 12. a 18. měsíci. Incidence FK v USA a Evropě se udává mezi 2-5 %. FK jsou rozděleny do tří typů: nekomplikované, komplikované a febrilní status epilepticus. Nekomplikované FK jsou generalizované, trvají <15 minut a neopakují se do 24 hodin. Komplikované FK trvají déle (>15 minut), mají fokální charakter a opakují se do 24 hodin. Febrilní status epilepticus je nejtěžší typ FK, který trvá >30 minut. Kromě zvýšeného rizika vzniku epilepsie a psychiatrických onemocnění patří mezi nejčastější komplikace samotných FK rekurentní febrilní křeče (RFK). Celkově se riziko recidivy snižuje s věkem z 50 % u dětí mladších 12 měsíců při první atace FK na 30 % u dětí starších. Kromě nízkého věku patří mezi další rizikové faktory pozitivní rodinná anamnéza pro FK a krátké trvání infektu před prvními křečemi. Ve snaze předejít vzniku FK a RFK bylo zkoumáno několik potenciálních prediktorů, např. maximální tělesná teplota, laboratorní hodnoty metabolismu železa, dysbalance elektrolytů, věk, pohlaví. Cílem naší první studie bylo identifikovat možné prediktory rozvoje FK ve snaze předejít jejich vzniku či opakování. Cílem přehledného referátu bylo nastínit roli neurozánětu v etiopatogenezi epilepsie a související potenciální nové terapeutické možnosti založené na modulaci zánětlivé odpovědi. Druhá studie se věnuje významu neurozánětu v epileptogenezi a rozvoji farmakorezistence u dětských pacientů s epilepsií ve snaze najít nové možnosti cílené „personalizované“ terapie epilepsie.

Abstract:

Febrile seizures (FS) represent the most common type of convulsions in children. They typically occur in children between 6 months and 5 years of age with a peak at 12-18 months. The incidence of FS in the US and Europe is reported to be between 2-5 %. FS are divided into three types: simple, complex and febrile status epilepticus. Simple FS are generalized, last <15 minutes, and do not recur within 24 hours. Complex FS last longer (>15 minutes), are focal in nature and recur within 24 hours. Febrile status epilepticus is the most severe type of FS, lasting >30 minutes. In ad-dition to the increased risk of developing epilepsy and psychiatric diseases, recurrent febrile seizures (RFS) are among the most common complications of FS themselves. Overall, the risk of recurrence decreases with age from 50 % in children younger than 12 months at the first attack of FS to 30 % in older children. In addition to young age, other risk factors include a positive family history for FS and a short duration of infection before the first convulsions. In an attempt to prevent the occurrence of FK and RFS, several potential predictors were investigated, e.g. maximum body temperature, laboratory values of iron metabolism, electrolyte imbalance, age, gender. The aim of our first study was to identify possible predictors of FS development in an attempt to prevent their occurrence or recurrence. The aim of the comprehensive report was to outline the role of neuroinflammation in the etiopathogenesis of epilepsy and related potential new therapeutic options based on the modulation of the inflammatory response. The second study focuses on the importance of neuroinflammation in epileptogene-sis and development of pharmacoresistance in pediatric patients with epilepsy in an effort to find new possibilities for targeted "personalized" epilepsy therapy.
