Subjective perception of treatment in patients with multiple myeloma

Abstract:

Cíl: Cílem práce bylo zhodnotit toleranci a vnímání léčby vysokodávkovanou chemoterapií s autologní transplantací periferních krvetvorných buněk (PBSC) a tandemovou transplantací u pacientů s mnohočetným myelomem v období hospitalizace od přijetí až do propuštění a krátce po propuštění do domácího ošetřování. Metodika: Data byla sbírána formou dvou dotazníků vytvořených pro toto průzkumné šetření na pracovišti Kliniky hematoonkologie Fakultní nemocnice Ostrava. První soubor tvořila skupina pacientů, kteří podstoupili vysokodávkovanou chemoterapii s autologní transplantací PBSC. Druhou skupinu tvořili pacienti, kteří podstoupili také tandemovou transplantaci. Kritériem výběru byla léčba první linie mnohočetného myelomu, zakončená vysokodávkovanou chemoterapií s autologní transplantací PBSC, provedena ve Fakultní nemocnici Ostrava. Výsledky: Šetřením bylo zjištěno, že dobrý psychický stav má pozitivní vliv na průběh a prožívání procesu transplantace krvetvornými buňkami. Pouze 13 % pacientů se vyjádřilo, že měli psychické potíže jako strach a úzkost z toho co přijde, což taky ovlivnilo jejich ochotu podstoupit proces znovu, případně tento postup doporučit. Mezi největší potíže označili v pořadí nechutenství, zažívací potíže, psychické strádání a horečku. Závěr: Plná informovanost pacienta o celém procesu a včasná preventivní opatření vedou ke zmírnění nežádoucích účinků vysokodávkované chemoterapie a zlepšení celkové psychické pohody. Při indikaci procesu autologní transplantace krvetvorných buněk je nezbytné zhodnotit nejen fyzický stav, ale také psychickou stránku pacienta, která může výrazně ovlivnit vnímání celého procesu.

Abstract:

Goal: The main goal of the thesis was to evaluate tolerance and perception of treatment by high-dose chemotherapy in combination with peripheral blood stem cell autologous transplantation and tandem transplantation in patiens with multiple myeloma in the period from hospital admission to release and the next days at home. Methodology: The data was collected by two questionaires created specifically for this research on Department of Haematooncology, University Hospital Ostrava. The first group contains patients who were treated by high-dose chemotherapy in combination with peripheral blood stem cell autologous transplantation. The second group contains these patients who were treated by tandem transplantation. Inclusion criteria included patients with multiple myeloma treated by the first line of chemotherapy followed by autologous transplantation of PBSC in University Hospital Ostrava. Results: Good psychological condition has positive effect for the process and experiencing of hematopoietic stem cell transplantation. Only 13 % of patients said they had a fear and anxiety about what was coming, which also affected their willingness to repeat the process or recommend this procedure. The biggest problems of patients contained loss of appetite, digestive problems, psychological distress and fever. Conclusion: Full patient awareness of the entire process and timely preventive measures go along with alleviation the adverse effect of high dose chemotherapy and improve overall mental well-being. It is necessary to evaluate not only the physical condition but also the mental aspect of the patient, which can significantly affect the perception of the process.
