Abstract:

Cílem této disertační práce je prozkoumat možnosti psychoterapeutické léčby u pacientů s medicínsky nevysvětlenými somatickými symptomy (MUPS). Největší důraz v teoretické části disertace je na prozkoumání možností psychoterapeutické léčby u pacientů s MUPS. Disertace se skládá ze 3 studií. Dvě z nich jsou empirické. Vzorek pacientů pochází ze sedmi klinik napříč Českou republikou. Třetí studie je systematická rešerše mechanismů změny. Studie A je zaměřena na psychoterapeutickou změnu po skupinové psychoterapii. Jedno z hlavních zjištění je, že na konci psychoterapie pacienti udávali malou změnu v rámci intenzity somatických symptomů. Studie B je systematický přehled mechanismů změny, které se ve výzkumných studiích objevují ve vztahu k psychoterapii pacientů s MUPS. Studie C je zaměřena na negativní zkušenosti pacientů po skupinové psychoterapii. Nejčastější negativní zkušenosti pacientů byla zhoršení se v psychických symptomech. Přínosem této práce je, že řeší aktuální fenomény v rámci psychoterapeutického procesu pacientů s MUPS.

Abstract:

The aim of the dissertation was to explore the possibilities of psychotherapy in patients with medically unexplained somatic symptoms (MUPS). The greater part of the theoretical background is then devoted to the possibilities of psychological treatment of patients with MUPS. The dissertation itself contains three studies. Two of them are empirical and are based on a sample of patients with MUPS from clinical sites across the Czech Republic. The third study is a systematic literature review. Study A focuses on the effectiveness of group-based psychotherapy. One of the main findings is that there was a low change in somatic symptoms after the end of treatment in patients with MUPS. Study B provides a systematic overview of the mechanisms of change used in psychotherapy process research in MUPS patients. Study C looked at the negative effects of group-based psychotherapy. The most common negative effect of psychotherapy in MUPS patients was the worsening of both mental and physical symptoms. The dissertation thus points to some important phenomena, whether in relation to the process of psychotherapy or to the actual results of psychotherapy in patients with MUPS.
