Physiotherapy in amateur cyclists with back pain

Abstract:

Bakalářská práce se zabývá problematikou bolestí páteře u amatérských cyklistů. Cílem práce je diagnostikovat svalové dysbalance u amatérských cyklistů s bolestmi zad a navrhnout možnosti jejich korekce. Práce je rozdělena na část teoretickou a praktickou. V teoretické části je popsána obecná charakteristika cyklistiky, anatomie lidské páteře, svalových vláken a svalů, biomechanika a kineziologie jízdy na kole. Dále se v práci nacházejí kapitoly zabývající se svalovými dysbalancemi, bolestí, a to především bolesti zad, a možnostmi řešení bolesti zad jako následek svalových dysbalancí. Praktická část zahrnuje kazuistiky dvou amatérských cyklistů, u kterých byl proveden kineziologický rozbor, kde se odhalila přítomnost svalových dysbalancí a špatných pohybových stereotypů. Na základě vyšetření se stanovil cíl terapie spolu s krátkodobým a dlouhodobým plánem fyzioterapie. Z konceptů popsaných v teoretické části jsem pro tuto práci zvolil metodu Dynamické Neuromuskulární Stabilizace, ze které jsem čerpal cviky do cvičební jednotky. Terapie trvala celkem 3 měsíce, přičemž probandi cvičili zadané cviky šestkrát týdně přibližně 20 minut. Během této doby došlo k sedmi intervencím, kdy se postupně upravovali a modifikovali zadané cviky od lehčích k obtížnějším. Po skočení terapie se ve výstupním vyšetření u obou probandů potvrdil pozitivní účinek kompenzačního cvičení na odstranění svalových dysbalancí a na eliminaci bolesti zad. Došlo ke zlepšení držení postury, zlepšení trupové stabilizace, zvýšení celkové mobility páteře, eliminaci bolesti zad, posílení oslabených a uvolnění přetížených svalových skupin.

Abstract:

The bachelor thesis deals with the problem of spinal pain in amateur cyclists. The aim of the thesis is to diagnose muscular imbalances in amateur cyclists with back pain and to suggest possibilities of their correction. The work is divided into theoretical and practical parts. The theoretical part describes general characteristics of cycling, anatomy of the human spine, muscle fibres and muscles, biomechanics and kinesiology of cycling. Furthermore, the thesis contains chapters dealing with muscle imbalances, pain, especially back pain, and the possibilities of solving back pain as a consequence of muscle imbalances. The practical part includes case studies of two amateur cyclists who underwent kinesiological analysis, which revealed the presence of muscle imbalances and poor movement stereotypes. Based on the examination, a goal of therapy was established along with a short and long term physiotherapy plan. From the concepts described in the theoretical part, I chose the Dynamic Neuromuscular Stabilization method for this work, from which I drew the exercises for the exercise unit. The therapy lasted a total of 3 months, with probands practicing the specified exercises six times a week for approximately 20 minutes. There were seven interventions during this time, with progressive adjustments and modifications of the assigned exercises from easier to more difficult. At the end of the therapy, the outcome examination of both probands confirmed the positive effect of the compensatory exercises on the elimination of muscle imbalances and on the elimination of back pain. There was an improvement in postural alignment, improvement in trunk stabilization, increase in overall spinal mobility, elimination of back pain, strengthening of weakened and relaxation of overloaded muscle groups.
