The appearance of various types of postcranial skeletal fractures on postmortem CT record

Abstract:

Hlavním tématem práce bylo využití CT a objemového zobrazení při hodnocení traumat postkraniálního skeletu z post-mortem vyšetření. Práce poskytuje přehled o charakteristice jednotlivých fraktur podle příčin a mechanismů vzniku. Rovněž popisuje vyšetření pomocí počítačové tomografie a jeho interdisciplinární využití. Výčet informací o CT vyšetření poskytuje přehled jak o jeho výhodách, tak i nevýhodách a o tom, s jakými omezeními se můžeme setkat v praxi. CT záznamy byly poskytnuty z archivu Ústavu soudního lékařství v Hradci Králové. Jejich vizualizace bylo dosaženo s použitím softwaru Amira 5.3.3, který nám při vhodném nastavení parametrů umožnil získání vhodného obrazu fraktur. S pomocí vytvořeného klasifikačního systému byly tyto fraktury rozděleny podle typu a lokalizace na jednotlivých částech skeletu. Pro vytvoření atlasu byla k systému klasifikace přiřazena jednotlivá vyobrazení fraktur. Vyhodnoceno bylo celkem 40 případů rozdělených do 7 kategorií příčin smrti, z nichž jedna zahrnovala případy bez určené příčiny. Posouzení, zda byla poranění typická pro jednotlivé kategorie, bylo možné u dopravních nehod a případů oběšení, u kterých byl dostatečný počet případů s traumaty. Vyhodnocení zbylých kategorií úmrtí se jeví jako neprůkazné z důvodu nedostatečného množství dat.

Abstract:

The main focus of this study was the use of CT and volume imaging in the evaluation of postcranial skeletal trauma from post-mortem examination. The thesis provides an overview of the characteristics of individual fractures according to their causes and mechanisms of occurrence. It also describes computed tomography examination and its interdisciplinary use. A list of information about CT examination provides an overview of both its advantages and disadvantages and the limitations that can be encountered in practice. CT records were provided from the archives of the Institute of Forensic Medicine in Hradec Králové. Their visualization was achieved using the Amira 5.3.3 software, which, with appropriate parameter settings, allowed us to obtain a suitable image of the fractures. With the help of the developed classification system, these fractures were classified according to type and localization on individual parts of the skeleton. To create an atlas, the individual fracture images were assigned to the classification system. A total of 40 cases were evaluated, divided into 7 cause-of-death categories, one of which included cases without an identified cause. Assessment of whether the injuries were typical of each category was possible for road traffic accidents and hangings, where there were a sufficient number of cases with trauma. Evaluation of the remaining categories of deaths appears inconclusive due to insufficient data.
