Abstract:

Hlavním cílem této výzkumné práce je popsat péči o ţenu v šestinedělí poskytovanou komunitními porodními asistentkami v ČR a USA. Tento hlavní výzkumný cíl lze rozdělit do čtyř následujících dílčích výzkumných cílů: 1. Zjistit rozdíly v práci komunitních porodních asistentek v ČR a USA 2. Srovnat problémy ţen po porodu v ČR a v USA 3. Zjistit kolik ţen po porodu v domácím prostředí navštíví za 1 den porodní asistentka v ČR a v USA 4. Porovnat problémy v práci komunitních porodních asistentek v ČR a USA Metodika: Realizované výzkumy zabývající se péčí o ţenu v šestinedělí poskytovanou porodními asistentkami vyuţívají metody kvantitativního výzkumu. Proto je i v tomto případě zvolena technika sběru dat pomocí nestandardizovaného dotazníku. Byla vypracována česká a anglická verze. Dotazníky obsahují stejné otázky, některé jsou upraveny aktuálně pro daný stát. Jedná se o dotazník vlastní konstrukce v rozsahu 2 stran o 28 otázkách, který je uloţen v příloze práce. Je rozdělen do 4 skupin: V 1. skupině je 6 otázek, které charakterizují respondentky účastnící se výzkumu. 2. skupina je zaměřena na návštěvy komunitních porodních asistentek u ţen po porodu. Obsahuje 6 otázek. 3. skupina popisuje ţenu v šestinedělí pomocí 6 otázek. Ve 4. skupině je 10 otázek, které se týkají zaměstnání komunitní porodní asistentky. Porovnání výsledků má ukázat rozdíly v činnostech, které vykonávají porodní asistentky v daných státech. Výsledky: Na základě provedených výpočtů lze konstatovat, ţe v práci komunitních porodních asistentek ţijících v ČR existují statisticky významné rozdíly v poskytovaných sluţbách ve srovnání s prací komunitních porodních asistentek v USA. Na základě statistických výpočtů lze konstatovat, ţe v práci komunitních porodních asistentek ţijících v ČR existují statisticky významné rozdíly v poskytování péče ve srovnání s prací komunitních porodních asistentek v USA. Ze zjištěných údajů lze tedy vyvozovat, ţe výskyt problémů u ţen po porodu není závislý na státě, ve kterém ţena ţije. Na základě statistických výpočtů lze konstatovat, ţe porodní asistentky v ČR navštíví za jeden den statisticky rozdílný počet ţen po porodu neţ porodní asistentky v USA. Na základě statistických výpočtů lze konstatovat, ţe v pracovních problémech komunitních porodních asistentek ţijících v ČR, existují statisticky významné rozdíly ve srovnání s pracovními problémy komunitních porodních asistentek v USA. 6 Závěr: V teoretické části práce byla popsána historie a současnost v porodní asistenci v naší zemi a v Pennsylvánii. V praktické části je na základě dotazníkového řešení popsána péče o ţenu v šestinedělí v obou zkoumaných zemích. Výsledky empirického šetření ukázaly rozdíly v péči o šestinedělku vykonávanou komunitními porodními asistentkami v závislosti na státě, ve kterém ţena ţije. Nabízí se nám zde moţnost inspirace v USA pro zavedení systému péče komunitních porodních asistentek s přihlédnutím k aktuálním specifikám v Česku. Jestliţe má dojít ke zlepšení péče o ţenu v šestinedělí v ČR, pak je nutné apelovat na MZČR ve věci úpravy legislativy tak, aby porodní asistentky mohly pracovat nezávisle na lékařské indikaci tak, jak jim umoţňují kompetence, znalosti a dovednosti získané svým vzděláním deklarované v Mezinárodní definici porodní asistentky.

Abstract:

The main aim of this research work is to describe the care of women during the postpartum period provided by the community midwives in the Czech Republic and the United States. The main aim can be divided into four sub-aims as follows: 1. To find out the differences between the work of the community midwives in the Czech Republic and the United States 2. To compare the problems of women after the delivery in the Czech Republic and the United States 3. To find out how many women are provided the home visiting service by the midwives in the Czech Republic and the United States during 1 day 4. To compare the problems the community midwives face during their work in the Czech Republic and the United States Methodology: The implemented studies dealing with the care of women during the postpartum period provided by the community midwives have used the methods of quantitative research. Therefore the technique of data gathering by the means of a non-standard questionnaire was chosen for this study. Czech and English versions of the questionnaire were made containing the same questions, some of them being modified for the given country. The self-designed questionnaire is 2 pages long and includes 28 questions and it is enclosed in the appendix. The questionnaire is divided into 4 groups: In the first group, there are 6 questions which characterize the respondents taking part in the research. The second group is focused on the home visiting service of the community midwives at the women after the delivery. It also includes 6 questions. The third group describes the women during the postpartum period by the means of 6 questions. In the fourth group, there are 10 questions concerning the job of community midwife. Result comparison is supposed to show the differences in the activities performed by the midwives in given countries. Results: Based on the carried out evaluation we can claim that there are statistically significant differences in the work and services provided by the community midwives living in the Czech Republic and the US. The data found out in the research show that the occurrence of problems in women after the delivery does not depend on the countries where they come from. On the basis of the statistical evaluation, it is possible to claim that the midwives in the Czech Republic visit a different number of women during a day compared to midwives in the US. Based on the statistical evaluation there are statistically 8 significant differences concerning the occupational problems of community midwives living in the Czech Republic and the US. Conclusion: The theoretical part of the thesis includes the history and present of the midwifery care in our republic and in Pennsylvania. In the practical part, the care of women during the postpartum period in both countries is described based on the questionnaire research. The results of the empirical research have shown differences in the care of the postpartum women provided by the community midwives depending on the country where the women come from. We may be inspired by the situation in the US to introduce a system of midwifery care in our republic taking account of the current specifics. If the care of women in the postpartum period in the Czech Republic is to be improved, it is necessary to appeal to the Ministry of Health of the Czech Republic to change the legislation, so the midwives could work independently of the medical indication according to their competences, knowledge and skills gained during their education declared in the International definition of a midwife.
