Uzlinový status pacientů se skvamózním karcinomem dutiny ústní a jeho vztah k vyšetření sentinelové uzliny

Nodal status of patients with squamous cell carcinoma of the oral cavity and its relation to sentinel node examination

Abstract:

Dlaždicobuněčný karcinom dutiny ústní se řadí mezi tumory s poměrně agresivním průběhem a častou generalizací do regionálních lymfatických uzlin. Metastatické postižení lymfatických uzlin se tak řadí mezi nejvýznamnější prognostické faktory a jejich prokázání může významně ovlivňovat průběh léčby pacienta. Jednou z možností histologického průkazu metastáz do lymfatických uzlin je vyšetření způsobem používaným při vyšetření sentinelové uzliny. V této bakalářské práci bylo provedeno nové histologické zpracování lymfatických uzlin způsobem typickým pro sentinelové uzliny u pacientů s negativním uzlinovým statusem s cílem odhalit přítomnost metastáz u těchto pacientů, které nebyly zjištěny klasickým zpracováním. V praktické části byl využit soubor 38 pacientů s karcinomem dutiny ústní a blízkých oblastí, kteří v letech 2017-2020 podstoupili blokovou krční disekci a byli diagnostikování bez metastatického postižení lymfatických uzlin. Lymfatické uzliny byly nově zpracovány extenzivním prokrajováním do celkové hloubky 1 mm s následným obarvením hematoxylinem a eozinem. Celkem bylo zpracováno 414 lymfatických uzlin. Metastatické postižení bylo prokázáno v 1 lymfatické uzlině (0,24 %). Velikost metastázy byla 2,907 mm, jednalo se o makrometastázu. Vyšetření způsobem sentinelové uzliny u pacientů s karcinomem dutiny ústní nemá, podle výsledku této práce, jednoznačně vyšší senzitivitu oproti klasickému zpracování. Přesto některé studie naznačují, že tento způsob vyšetření je při diagnostice krčních metastáz nezbytný a mělo by se mu v rámci včasné diagnostiky a léčby pacientů věnovat více pozornosti.

Abstract:

Squamous cell carcinoma of the oral cavity belongs to tumors with a relatively aggressive course and frequent generalization to regional lymph nodes. Thus, metastatic lymph nodes involvement rank among the most important prognostic factors and their demonstration may significantly affect the course of patient treatment. One of the options for histological demonstration of lymph node metastasis is the evaluation in the manner used in sentinel node examination. In this bachelor thesis, a new histological examination of lymph nodes was performed in a manner typical for sentinel lymph nodes in patients with negative nodal status in order to detect the presence of lymph node metastases in these patients that were not detected by conventional processing. In the practical part, a cohort of 38 patients with carcinoma of oral cavity and nearby areas who underwent block neck dissection between 2017 and 2020 and were diagnosed without metastatic lymph node involvement was used. The lymph nodes were reprocessed by extensive lymph node step-section to a total depth of 1 mm followed by hematoxylin and eosin staining. A total of 414 lymph nodes were treated. Metastatic involvement was demonstrated in 1 lymph node (0.24%). The size of the metastasis was 2.907 mm and it was a macrometastasis. According to the results of this study, sentinel node examination in patients with oral cavity cancer does not have a clearly higher sensitivity compared to conventional treatment. Nevertheless, some studies suggest that this type of examination is essential in the diagnosis of cervical metastases and should be given more attention in the early diagnosis and treatment of patients.
