Physiotherapy after breast surgery at women

Abstract:

Úvod: Jako téma pro svou bakalářskou práci jsem si vybrala fyzioterapii u žen po operaci prsu. K výběru mne dovedla osobní rodinná zkušenost a aktuálně zvyšující se procento žen s touto diagnózou. Cíl práce: Cílem mé bakalářské práce je shrnutí poznatků o karcinomu prsu a přispět k řešení problematiky fyzioterapie, především volby vhodných fyzioterapeutických postupů, metod a konceptů u pacientky po operaci prsu. Metodika: Spolupráce s mou pacientkou začala 4 měsíce po její operaci a trvala 4 měsíce. Nejprve jsem provedla vstupní vyšetření včetně kineziologického rozboru dle Jandy - odebrání anamnézy, statické držení těla, konfigurace, trofika, hybnost, antropometrie, goniometrie, vyšetření svalové síly a zkrácených struktur, vyšetření měkkých tkání, kloubní vůle a v neposlední řadě vyšetření pohybových stereotypů. Na základě vstupního vyšetření jsem stanovila krátkodobý fyzioterapeutický plán s využitím konkrétních terapeutických postupů, konceptů a metod (polohování, techniky a mobilizace měkkých tkání, respirační fyzioterapie, cévní gymnastika, kondiční cvičení, cvičení s pomůckami, PIR, PNF, Brügger koncept?, ACT?, Spiraldynamik?). Následně jsem provedla kontrolní vyšetření po 4 týdnech. Po ukončení společné terapie po 12 týdnech jsem provedla realizaci výstupního vyšetření, na základě kterého jsem stanovila dlouhodobý fyzioterapeutický plán, včetně cvičební jednotky. Na konci porovnávám a hodnotím výsledky ze vstupního, kontrolního a výstupního vyšetření. Závěr: Při výstupním kineziologickém vyšetření jsem zaznamenala několik změn, a to především ve zlepšení v oblasti měkkých tkání (ke zlepšení stavu jizvy, odstranění patologických bariér ve fasciích). Dále došlo ke zvýšení svalové síly, k obnově správného stereotypu dýchání a stereotypu pohybu (abdukce v ramenním kloubu). Největší změna nastala ve zlepšení ROM do FL a ABD v ramenním kloubu (do flexe ze 170° na 180°, do abdukce ze 140° na 170°). Fyzioterapeutická intervence tak pozitivně ovlivnila stav paci-entky po operaci prsu a urychlila její rekonvalescenci.

Abstract:

Introduction: I have chosen the physiotherapy for women after the breast surgery as a topic for my bachelor thesis in accordance with the personal experience in our family and the recent percentage increase of women with such a diagnosis. Purpose: The aim of my bachelor thesis is to summarize the knowledge of the breast cancer and to contribute the solution of physiotherapy, especially the choice of appropriate physiotherapeutic procedures, methods and concepts for the patient after the breast surgery. Methodology: The cooperation with my patient have started 4 months after her surge-ry and it have taken 4 months. At first, I have provided an entry examination including the Janda kinesiological analysis - taking a case history, static pose, configuration, trophics, mo-mentum, anthropometry, goniometry, an examination of the muscle strength and shortened structures, an examination of the soft tissues, joint clearance and finally an examination of the movement stereotypes. According to the entry examination I have determined a short-term physiotherapeutic plan using the specific therapeutic procedures, concepts and methods (posi-tioning, soft tissue mobilization and techniques, respiratory physiotherapy, vascular gym-nastics, fitness exercises, exercises with aids, PIR, PNF, Brügger concept?, ACT? , Spiral-dynamik?). After 4 weeks I have carried out a check-up. After completion of the 12-week co-therapy I have provided the realization of the exit examination to establish the long-term phy-siotherapy plan, including an exercise unit. At the end I am comparing and evaluating results from the entry, check-up and exit examination. Conclusion: During the exit kinesiological examination I have noticed several changes, especially in the area of the soft tissues (improving the scar condition, removing the pathological barriers in fascia). Furthermore, there was an increase in the muscle strength, the restoration of the correct breathing stereotype and the stereotype of movement (abduction in the shoulder joint). The biggest change occurred in the improvement of ROM into FL and ABD of the shoulder joint (into flex 180 ° from 170 °, into the abduction 170° from 140 °). The physiotherapeutic intervention thus positively affected the patient's condition after the breast surgery and accelerated her recovery.
