Comprehensive physiotherapy after ablation of the breast,the therapy of lymphedema

Abstract:

Úvod: Tématem mé bakalářské práce je komplexní fyzioterapie po ablaci prsu s přihlédnutím na terapii sekundárního lymfedému. Seznámením se s novými poznatky jsem se snažila souhrnně uchopit problematiku karcinomu prsu a přiblížit závažnost této diagnózy. Cíl práce: Cílem práce je přispět k řešení problematiky fyzioterapie správnou volbou fyzioterapeutických postupů, konceptů a metod u pacientky po mastektomii. Metodika: Hlavní část mé bakalářské práce popisuje kazuistiku pacientky po ablaci prsu. Spolupracovali jsme intermitentně od 3. týdne po operaci po dobu 11 měsíců. Před zahájením fyzioterapeutické intervence jsem provedla vstupní kineziologické vyšetření (vyš. aspekcí a palpací, goniometrie HKK, antropometrie HKK a hrudníku,vyš. zkrácených struktur a pohybových stereotypů), na jehož základě jsem vypracovala krátkodobý rehabilitační plán. V průběhu fyzioterapeutické léčby jsem provedla dvě kontrolní vyšetření po dvou a pěti měsících. Náplní cvičebních jednotek bylo využití fyzioterapeutických konceptů a metod (polohování, respirační fyzioterapie, cévní gymnastika, mobilizace měkkých tkání, PIR, PNF, DNS, BPP, Brüger ? koncept, cvičení s pomůckami a jiné) k ošetření měkkých tkání, nácviku správného dechového stereotypu a dynamiky dýchání s cílem obnovit mobilitu hrudníku, zlepšení rozsahu pohybu a svalové síly v ramenním kloubu jak izolovaně, tak i v rámci posturálně lokomoční funkce. Před ukončením léčby jsem provedla výstupní kineziologické vyšetření, kdy výsledky vyšetření mi pomohly ke stanovení dlouhodobého rehabilitačního plánu. Výsledky: Viditelné zlepšení jsem zaznamenala v posunlivosti a protažitelnosti jizvy, fascie a okolní měkké tkáně. Nejvýraznější změna nastala v hybnosti RK, především do FL, ABD a ZR v ramenním kloubu (FL z 95° na 180°, ABD z 80° na 170°, ZR 10° na 85°). Závěr: Závěrem mé práce jsem zhodnotila výsledky vstupního a výstupního vyšetření. Fyzioterapeutická intervence měla pozitivní vliv na život pacientky.

Abstract:

Introduction : The topic of this bachelor thesis is complex physiotherapy after breast ablation with regard to the therapy of secondary lymphedema. By studing new findings I tried to summarize the issue of breast cancer and describe the severity of this diagnosis. The goal of the thesis : The goal of this thesis is to contribute and to solve issues concerning physiotherapy by the right choice of physiotherapeutic processes, concepts and methods in patients after mastectomy operation. Methodology: In the main part of the thesis I describe a case report the patient after recovering from breast ablation. I was providing care for the patient from the 3rd week after surgery occasionally for 11 months. Before we started the physiotherapy intervention I had performed a kinesiological examination (aspection and palpation examination, goniometry and anthropometry of upper limbs and thorax, examination of shortened structures and stereotypical movements) on the basis of which I prepared a short-term rehabilitation plan. During the physiotherapy treatment I performed two follow-up examinations - the first after two months and the second after five months. Each exercise session consisted of specific physiotherapeutic concepts and methods such as: positioning, respiratory physiotherapy, vascular gymnastics, soft tissue mobilization, PIR, PNF, DNS, BPP, Brüger ? concept, exercise with other equipment etc. All these methods helped to treat soft tissues, practice correct breathing technique and dynamics, to restore chest mobility, to improve range of motion and muscle strength in the shoulder joint both in isolation and postural locomotion function. Before the end of the treatment I performed the final kinesiological examination. I created a long-term rehabilitation plan based on the results of the final examination. Results: I noticed a significant improvement in the displaceability and elongation of the scar, fascia and surrounding soft tissue. The most obvious change occurred in the range of montion of shoulder joint, mainly into FL, ABD and ER - (FL 95 180 deg., ABD 80 170 deg., ZR 10 85 deg.). Conclusion: At the end of the thesis I evaluated the results of entry and last examination. This physiotherapeutic intervention had a positive effect on the quality of the patient's life.
