Abstract:

Tato diplomová práce se věnuje problematice koordinované rehabilitace u klientů po cévní mozkové příhodě a problematice multidisciplinárních týmů. Cílem této práce je popsat a zmapovat současnou situaci týkající se možností koordinované rehabilitace pro tyto klienty, tedy jak tito klienti vnímají komunikaci s personálem v nemocnicích a zda mají veškeré potřebné informace k dispozici. Zároveň mapuje situaci očima lékařů, kteří v rámci nemocnic jednotlivé složky koordinované rehabilitace předepisují. Teoretická část práce se věnuje problematice cévní mozkové příhody, je zaměřena na příhody ischemické, jejich diagnostice, léčbě, prevenci. Druhá kapitola je zaměřena na pojem koordinované rehabilitace u klientů po cévní mozkové příhodě, dále na teoretické poznatky o principech fungování multidisciplinárního týmu zaměřeného na tyto klienty a charakterizuje jednotlivé členy týmu a jejich kompetence včetně nejmodernějších principů jejich terapie. Výzkumná část je zaměřena na metodologii výzkumu a následnou interpretaci dat. Cílem je najít kritická místa fungování multidisciplinárních týmů v rámci nemocnic v hospitalizační fázi po prodělané ischemické cévní mozkové příhodě. Dále definovat jaké potřeby týkající se jednotlivých složek koordinované rehabilitace klienti mají, zda mají dostatek informací o možnostech komplexní rehabilitace a zda jim je tato rehabilitace poskytována. Je zaměřena i na pohled lékařů - co je ovlivňuje v předepisování jednotlivých složek koordinované rehabilitace, jak komunikují s kolegy v rámci multidisciplinárního týmu a zda využívají možností následné péče. Výzkum je zpracován metodou kvantitativního výzkumu pomocí nestandardizovaných dotazníků s uzavřenými a polozavřenými odpověďmi. Data jsou statisticky zpracována tříděním prvního stupně a statistickým ověřením hypotéz pomocí chí-kvadrát testu, Kruskal-Wallisova testu, Friedmanovy ANOVY, Kendallova koeficientu shody v programu STATISTICA 10 CZ.

Abstract:

This thesis deals with the issue of coordinated rehabilitation of clients that have suffered a stroke and the issue of multidisciplinary teams. The aim of this study is to describe and map the current situation regarding the possibility of coordinated rehabilitation for these clients, namely how these clients perceive communication with the staff in hospitals and whether all necessary information is available. Also it monitors the situation through the eyes of doctors who prescribe individual components of the coordinated rehabilitation within hospitals. The theoretical part deals with the issue of the stroke, it is focused on ischemic strokes, their diagnostics, treatment and prevention. The second chapter focuses on the concept of coordinated rehabilitation for clients that have suffered a stroke. Furthermore it focuses on theoretical knowledge of the principles of functioning of multidisciplinary team focused on these clients and it characterises the individual team members and their competencies including the most modern principles of their therapy. The experimental part is focused on methodology of the research and subsequent interpretation of data. The goal is to find critical points of the functionality of multidisciplinary teams within hospitals during hospitalization phase folowing the ischemic stroke. Also to define what needs related to individual components of coordinated rehabilitation the clients have. Whether they have enough information about the possibilities of complex rehabilitation and whether is this rehabilitation provided to them. It also focuses on the view of the doctors - what influences them during prescribing of individual components of the coordinated rehabilitation, how they communicate with their colleagues within the multidisciplinary team and whether they use the opportunity of aftercare. The research has been processed through the method of quantitative reasearch using non-standardized questionnaires with closed and semi-closed answers. The data are statistically processed by sorting of the first stage and by statistical verification of hypothesis using the chi-square test, Kruskal-Wallis test, the Friedman ANOVA, the Kendall's coefficient of concordance in program STATISTICA 10 CZ.
