Lactation counselling and its benefits in community care

Abstract:

Cíl: Zjistit, co má vliv na úspěšnost kojení v prvních sedmi dnech po porodu. Metodika: Soubor respondentů tvořilo 135 párů žen a novorozenců do třetího dne po porodu a 114 párů žen a dětí do sedmého dne po porodu. Pro získání informací byla použita metoda nestandardizovaného dotazníkového šetření. Dotazníky byly distribuovány ve 4 porodnicích, v období od 13. 12. 2010 do 10. 3. 2011. Hypotézy byly testovány pomocí chí-kvadrát testu a Fisherova exaktního testu. Výsledky: 56 novorozenců (41,48 %) se po 24 hodinách chovalo neoptimálně, ve třetím dnu po porodu 34 novorozenců (25,19 %) a jen 17 novorozenců (14,91 %) se v sedmý den chovalo neoptimálně. Nadměrnou ztrátu hmotnosti třetí den po porodu utrpělo 9 novorozenců (6,67 %). 29 žen (21,48 %) třetí den po porodu uvedlo stejný nebo trochu plnější pocit plnosti v prsou. Významný vliv na úspěšnost kojení v prvních sedmi dnech po porodu má primiparita matek, ploché či vpáčené bradavky, délka I. a II. doby porodní, doba bez spánku více jak 18 hodin před porodem, počet kojení za den, tekutiny podané novorozenci do sedmého dne po porodu, pocit plnosti v prsou a chování novorozence u kojení. Neprokázali jsme významný vliv absolvováním předporodních kurzů a poporodní návštěvou zdravotníků v komunitě na chování novorozenců u kojení ani na problémy týkajících se kojení, které se vyskytly do sedmého dne po porodu. Závěr: Zjistili jsme, co má špatný vliv na úspěšnost kojení v prvním týdnu po porodu. Díky tomu můžeme účinněji poskytovat laktační poradenství a zvýšit počet kojících žen a tak zvýšit přínos laktačního poradenství v komunitní péči.

Abstract:

Aim: To find out what has an influence on the success of breast-feeding during the first seven days after the childbirth. Methodology: The group of respondents consisted of 135 mother-infant pairs with infants not more than three days old and 114 mother-infant pairs with infants not more than seven days old. The method of non-standardized questionnaire survey was used to obtain the information. The questionnaires were distributed in four maternity hospitals, in the period from 13. 12. 2010 to 10. 3. 2011. Hypotheses were tested by means of chi-square test and Fisher´s exact test. Results: After 24 hours 56 neonates (41.48 %) behaved in a suboptimal way, on the third day after the childbirth 34 neonates (25.19 %) behaved in a suboptimal way and on the seventh day only 17 neonates (14.91 %) behaved in a suboptimal way. 9 neonates (6.67 %) suffered from an excessive weight loss on the third day after the childbirth. 29 women (21.48 %) stated the same or a little fuller feeling in the breast on the third day after the childbirth. The factors which have a significant influence on the success of breast-feeding during the first seven days after the childbirth are primiparity of mothers, flat or inverted nipples, duration of stages I and II of labour, time without sleep more than 18 hours long before the childbirth, number of breast-feeding per day, fluids given to the neonate not more than seven days old, feeling of fullness in the breast and behaviour of the neonate at breast-feeding. We have proved a significant influence of the completion of antenatal courses and postnatal medical visit in the community neither on the behaviour of the neonates at breast-feeding nor on the problems concerning breast-feeding which occurred by the seventh day after the childbirth. Conclusion: We have found out what has a negative influence on the success of breast-feeding during the first week after the childbirth. Due to this fact we can provide lactation consultancy more effectively and increase the number of lactating women and thus enhance the contribution of lactation consultancy in the community care.
