Psychosocial factors of coping with diabetes mellitus

Abstract:

Diplomová práce se v rámci salutogenetického paradigmatu A. Antonovského zaměřuje na proaktivní zvládání chronického onemocnění a co konkrétně nemocným s diabetem pomáhá udržet si aktivní a produktivní život bez výraznějších komplikací a za udržení uspokojivé kvality života. Cílem bylo u skupiny aktivních sportujících diabetiků (N=60) v produktivním věku ověřit roli smyslu pro soudržnost při zvládání a úspěšném zpracování (přijetí) diabetes mellitus. Zjistili jsme, že Antonovského teoretický konstrukt, smysl pro soudržnost (SOC), moderuje důsledky vlivu stresu na zdraví, resp. je v negativním vztahu k depresi (r=-0,788, p<0,001) a distresu z diabetu (r=-0,591, p<0,001). Aktivní diabetici se silným smyslem pro soudržnost spíše dodržují optimální zdravotní návyky (r=0,564, p<0,001), zejména pak stravovací a preventivní. Oproti lidem se slabým smyslem pro soudržnost spíše využívají adaptivní zvládací strategie, tj. pozitivní pohled, zaměřenost na problém, minimalizace hrozeb (hodnoty r se pohybují mezi 0,333 a 0,624) a spíše vykazují vysokou míru psychické pohody (r=0,839, p<0,001). Orientační screening smyslu pro soudržnost a tím zhodnocení rizikových a protektivních faktorů u každého pacienta umožňuje v klinické praxi přesnější indikaci psychologické intervence.

Abstract:

In the context of Antonovsky’s salutogenic paradigm, the thesis focuses on the proactive coping with chronic disease – specifically, what helps patients with diabetes mellitus maintain satisfactory quality of life and active or productive life with no serious complications. The aim was to examine the moderating role of the sense of coherence in coping with diabetes and the successful acceptance of the disease in active working-age diabetics (N=60). The results show that Antonovsky’s theoretical construct, the sense of coherence (SOC), moderates the health outcomes of stress, ie. correlates negatively with depression (r=-0.788, p<0.001) and diabetes distress (r=-0.591, p<0.001). The active diabetics who have a strong sense of coherence are more likely to adhere to good health habits (r=0.564, p<0.001), healthy eating habits and prevention especially. In contrast to people with weak sense of coherence, they tend more to apply adaptive coping strategies, ie. positive appraisal, problem focus and threat minimization (the r values vary between 0.333 a 0.624), and show higher levels of psychological well-being (r=0.839, p<0.001). In clinical practice, screening of every patient’s sense of coherence – and thus assessment of his/her risk and protective factors – enables the more accurate psychological intervention.
