Abstract:

Cílem této práce je porovnání funkčního výsledku jednotlivých metod osteosyntéz i konzervativního řešení s ohledem na věk pacienta. V naší studii jsme si položili 3 otázky. 1. Jsou zlomeniny C 1.1, C 1.2, C 1.3, C 2.1 indikovány ke konzervativní terapii? 2. Jsou zlomeniny C 2.2, C 2.3, C 3.1, C 3.2, C 3.3 pod 65 let věku pacienta indikovány k osteosyntéze úhlově stabilní dlahou? 3. Jsou zlomeniny C 2.2, C 2.3, C 3.1, C 3.2, C 3.3 nad 65 let věku pacienta indikovány k implantaci hemiartroplastiky (CKP)? Cílem doplňkové studie bylo zhodnocení osteonekrózy hlavice u intraartikulárních zlomenin proximálního humeru řešených LCP dlahou PHILOS. Výsledky naší studie potvrzují složitost a nejednoznačnost postupu ošetřování intraartikulárních zlomenin proximálního humeru. Při indikaci k operačnímu řešení LCP dlahou bychom měli vždy zvážit případnou konzervativní terapii nebo implantaci hemiartroplastiky. Osteonekróza hlavice výrazně zhoršovala funkční výsledek. Relativně vysoké procento osteonekrózy hlavice v naší studii dokazuje složitost ošetřování intraartikulárních zlomenin proximálního humeru LCP dlahou. Chirurgické řešení osteonekrózy hlavice je velmi náročné a zdlouhavé a mnohdy nedosáhne očekávaného výsledku. Důležitou součástí terapie je spolupráce pacienta.

Abstract:

The aim of this study is to compare the functional results of operative treatment methods and conservative treatment with respect to the age of patients. In our study, we asked three questions. 1. Are fractures C 1.1, C 1.2, C 1.3, C 2.1 indicated for conservative therapy? 2. Are fractures C 2.2, C 2.3, C 3.1 C 3.2 C 3.3 in patients under the age of 65 indicated for osteosynthesis by locking compression plate? 3. Are fractures C 2.2, C 2.3, C 3.1 C 3.2 C 3.3 in patients over 65 years of age indicated for hemiarthroplasty implantation (CKP)? Additional aim of the study was the evaluation of humeral head osteonecrosis in intra-articular fractures of the proximal humerus treated by LCP plate PHILOS. The results of our study confirm the complexity and ambiguity of procedures for treatment of intra-articular fractures of the proximal humerus. In indication for surgery by LCP plate we always should consider the possible conservative therapy or implantation of hemiarthroplasty. Humeral head osteonecrosis significantly worse functional outcome A relatively high percentage of humeral head osteonecrosis in our study demonstrates the complexity of treating intra-articular fractures of the proximal humerus by LCP plate. Surgical treatment of humereal head osteonecrosis is a very difficult and time consuming and often does not reach the expected result. An important part of therapy is patient compliance .
