Analysis of the phenotype in the differentiation of hematopoietic stem cells to the B lymphocyte line in monoclonal gammopathies

Abstract:

Náplní diplomové práce byla flowcytometrická analýza subpopulací hematopoetických kmenových buněk a různých vývojových fází diferencujících B lymfocytů, včetně plazmatických buněk, u osob s monoklonálními gamapatiemi. V rámci analýz provedených u osob s MGUS a MM bylo zjištěno, že celkový počet HSC, včetně primitivních progenitorů, není pokročilostí MG ovlivněn, avšak již od stádia lymfoidních progenitorů dochází u MM k poruše diferenciace postihující časná stádia diferencujících B lymfocytů. Vyzrálejší formy B lymfocytů, včetně plazmablastů, pak nejsou u MM početně postiženy. Lze tedy konstatovat, že u pacientů s MM může existovat porucha diferenciace B lymfocytů, a to již na úrovni lymfoidních progenitorů, která je pravděpodobně zapříčiněna působky v mikroprostředí KD. Další studium hematopoetických kmenových buněk a diferencujících B lymfocytů u nově diagnostikovaných pacientů s MG či sledování obnovy krvetvorby po provedené autologní transplantaci periferních kmenových buněk může u osob s MG pomoci získat nové informace stran patofyziologie tohoto onemocnění nebo predikovat chování MG či léčebnou odpověď u MM.

Abstract:

The aim of this thesis was flow cytometric analysis of hematopoietic stem cell subpopulations and different developmental stages of differentiating B lymphocytes, including plasma cells, in patients with monoclonal gammopathy. The analyses of MGUS and MM patients showed that the total number of HSCs, including primitive progenitors, is not affected by progress of monoclonal gammopathies (MG). However, starting with the lymphoid progenitor stage, there is a failure of differentiation affecting the early stages of differentiating B cells in MM. More mature forms of B lymphocytes and plazmablasts were not affected in MM. It can be concluded that in patients with MM, possibly there is a failure of differentiation of B cells, already at the level of lymphoid progenitors, which is probably caused by mediators in the microenvironment of the bone marrow. Further study of hematopoietic stem cells and differentiating B cells in newly diagnosed MG patients and monitoring of hematopoietic recovery after autologous transplantation by peripheral blood stem cells can help to obtain new information about pathophysiology of the disease or predict the behavior of MG or response to therapy in MM in these patients.
