Identification of genes predictive for response to chemotherapy in colorectal carcinoma of stage II from gene expression data.

Abstract:

V současné době je velkým fenoménem hledání molekulárně-biologických principů charakteristických pro různá nádorová onemocnění. Vzhledem k tomu, že kolorektální karcinom je jedním z nejrozšířenějších nádorových onemocnění ve vyspělých zemích, je středem zájmu mnoha klinických studií. Terapeutickým standardem pro pacienty s kolorektálním karcinomem ve stádiu II je operativní odstranění nádoru (Seifert, 2012). Chirurgické odstranění rizikové tkáně je možné kombinovat s další tzv. adjuvantní chemoterapií. Ve srovnání s vedlejšími negativními účinky je obecně přínos adjuvantní chemoterapie pro pacienty se stádiem II velmi malý. Avšak výsledky dosavadních klinických studií ukazují, že u pacientů s vysokým rizikem relapsu je přínos z adjuvantní chemoterapie o něco vyšší (Gray et. al., 2004). Tento předpoklad vede k tomu, že se v praxi vyhodnocuje několik prognostických markerů jako je např. špatná diferenciace u histologického vyšetření, perforace střev a jiné faktory, díky nimž se pacienti řadí do skupin s lepší či horší prognózou. Podle toho se jim pak doporučí adjuvantní chemoterapie. V této práci jsem se zaměřila na problematiku kolorektálního karcinomu stádia II. Konkrétně na hledání genů, jejichž exprese by predikovala odpověď na adjuvantní chemoterapii. Existují studie , například AVANT (De Gramont et. al., 2011), které se tímto tématem již v minulosti zabývaly. Pomocí Coxova modelu proporcionálních rizik (Cox, 1972) jsem se pokusila nalézt skupinu genů, která by mohla jednoznačně určit přínos adjuvantní chemoterapie pro daného pacienta. Výsledkem práce je sada 39 prognostických genů a sada 49 prediktivních genů. Na závěr bylo identifikováno 10 genů (FAM84A, CDX2, DACH1, RNF43, SLC12A2, TDGF1, SELENBP1, MLF1, CYP39A1 a CREB3L1), které predikují odpověď pacienta na adjuvantní chemoterapii.

Abstract:

Nowadays discovering molecular-biological principles characteristic for various cancer diseases is a great phenomenon. Colorectal cancer as one of the most common cancers occurring in developed countries is included in many clinical trials. The chirurgical resection is standard for patients with this disease at stage II. This first choice therapy is often combined with other adjuvant chemotherapy. The benefit from adjuvant chemotherapy for patient with stage II is really insignificant. But some clinical trials proved the benefit of adjuvant chemotherapy for patients with high risk of relapse. This assumption leads to the evaluation of other prognostic markers, as for example poor differentiation by histological examination, perforation of the gut etc. Based on these markers the patients are discriminated in two groups with good and poor prognosis. Adjuvant chemotherapy is recommended only for patients with poor prognosis. This work is focused on colorectal cancer stage II and the main goal is the identification of genes predictive for patient response to adjuvant chemotherapy. There are some clinical trials focused on this topic, for example QUASAR (Gray et. al., 2004), AVANT (De Gramont et. al., 2011). In the frame of this work I have used Cox proportional hazards model (Cox, 1972) to find the set of genes predicting the response of patients with disease at stage II to adjuvant chemotherapy. The work resulted in the set of 39 prognostic genes and set of 49 predictive genes. Finally, I have identified 10 genes (FAM84A, CDX2, DACH1, RNF43, SLC12A2, TDGF1, SELENBP1, MLF1, CYP39A1 a CREB3L1) that predict response of patients with colorectal cancer stage II to adjuvant chemotherapy.
