Dying from perspective of general nurses working in the intensive care

Abstract:

Cíl: Cílem bakalářské práce bylo zjistit, jaký mají všeobecné sestry pracující na odděleních intenzivní péče, pohled na umírání pacientů. Metodika: Průzkumný soubor tvořilo 118 všeobecných sester pracujících na JIP a ARO Městské nemocnice v Ostravě, Vítkovické nemocnice v Ostravě a Bílovecké nemocnice v Bílovci. Data byla sbírána nestandardizovaným dotazníkem v období od září do listopadu 2015. Výsledky: Šetřením bylo zjištěno, že se 57 % sester dokáže lépe vyrovnat s umíráním pacientů s narůstající délkou praxe. Při péči o umírajícího pacienta pociťují sestry občas lítost, smutek, úzkost, úlevu a vyčerpání, ale nikdy nemají pocit osobního nebo medicínského selhání. Ačkoli je péče o umírající nemocné nelehká, není to pro 89 % sester důvod ke změně pracoviště. Komunikaci s příbuznými umírajícího pacienta hodnotilo 75 % sester jako profesionálně náročnou, i přesto se alespoň polovina sester snaží komunikovat s rodinou při každé návštěvě. Na pracovištích, kde je umožněna spoluúčast rodiny při péči o umírajícího pacienta (81 %), uvedly sestry přínosnost této péče, avšak záleží na konkrétním přístupu rodiny. Na 84 % oddělení intenzivní péče je podle sester poskytována důstojná péče o umírající pacienty a proto 67 % sester neshledalo žádnou oblast péče o umírající nemocné, kterou by chtěly změnit. Závěr: Péče o umírajícího pacienta je součástí holistické péče, proto je důležité, aby docházelo k uspokojování všech potřeb. Úkolem sestry je respektování umírajícího pacienta jako lidské individuality, která má právo na důstojné umírání s empatický přístupem.

Abstract:

Objective: The goal of this bachelor thesis was to find out what point of view of dying patients, nurses working at the department of intensive care have. Methods: In this research participated 118 nurses working at the department of Anesthesiology, Resuscitation and Intensive Care in the City hospital in Ostrava, Vítkovice hospital in Ostrava and Bílovecká hospital in Bílovec. All the data were collected by non-standardized questionnaire from September 2015 to November 2015. Results: During the research was found out that 57 % of the nurses are able to cope with dying patients better with increasing length of practice. When caring about dying patient the nurses might sometimes feel sorrow, sadness, anxiety, relief and exhaustion but they never feel any personal or medical failure. Although the care of dying patients is uneasy, for 89% of the nurses it is not a reason to change the workplace. 75 % of nurses evaluated the communication with relatives of dying patient as professionally demanding, still, at least half of the nurses try to communicate with the family during each visit. At the workplace where there is a possibility of participation of the family during the care of dying patient (81 %), the nurses reported the benefits of this care, but it depends on the specific approach of the family. 84 % of the department of intensive care, according to the nurses, provides dignified care of dying patients and therefore 67 % of the nurses did not find any area of care of dying patients which they would like to change. Conclusion: The care of dying patient is part of the holistic care, so it is important that all needs are satisfied. The goal of the nurse is to respect dying patient as a human individuality that has the right of dignified dying with empathetic approach.
