Molecular pathophysiology of recurrent aphthous stomatitis

Abstract:

Recidivující aftózní stomatitida (RAS) je multifaktoriální zánětlivé onemocnění dutiny ústní, které je charakterizováno výsevem bolestivých erozí, případně vředů na orální sliznici. Etiopatogeneze RAS dosud není zcela objasněna, avšak důležitou roli zde hrají genetické faktory. V rámci disertační práce byla provedena rešerše genetického pozadí RAS a analýza vybraných variant v genech pro interleukiny, matrix metaloproteinázy, NOD-like receptor 3 a serotoninový transportér u pacientů s RAS a u zdravých kontrol v české populaci. Disertační práce je postavena na komentovaném souboru šesti publikovaných prací autorky. Jedná se o jeden přehledový článek popisující vrozenou náchylnost k RAS a pět genetických asociačních studií kontrol a případů. Pro stanovení jednonukleotidových polymorfizmů (SNPs) a polymorfizmů v počtu tandemových repetic (VNTRs) byly použity metody založené na polymerázové řetězové reakci (PCR). Z celkem devatenácti studovaných genů jsme s RAS asociovali pouze geny pro interleukin-10 (IL10), NOD-like receptor 3 (NLRP3) a serotoninový transportér (SLC6A4). V IL10 genu jsme jako rizikové varianty pro RAS vyhodnotili T alelu a TT genotyp SNP IL10 -819C/T (P < 0,05; OR = 1,63; 95% CI = 1,07–2,46 a P < 0,05; OR = 2,93; 95% CI = 1,19–7,24) a A alelu i AA genotyp SNP IL10 -592C/A (P < 0,05; OR = 1,73; 95% CI = 1,14–2,62 a P < 0,05; OR = 2,93; 95% CI = 1,19–7,24). Při analýze haplotypů byl haplotyp ACC IL10 (-1082A/G, -819C/T, -592C/A) zjištěn jako protektivní (P < 0,05; OR = 0,58; 95% CI = 0,37–0,91). Naopak haplotyp ATA IL10 (-1082A/G, -819C/T, -592C/A) se statisticky významně častěji vyskytoval u pacientů s RAS (P < 0,05; OR = 1,63; 95% CI = 1,07–2,50). Taktéž TT genotyp polymorfizmu NLRP3 rs4612666 byl v porovnání s genotypy CT + CC u pacientů s RAS frekventovanější (P < 0,05; OR = 14,69; 95% CI = 1,73–124,72). V SLC6A4 genu jsme nalezli asociaci mezi haplotypem LA12 SLC6A4 (5-HTTLPR, rs25531, STin2) a zvýšeným rizikem RAS (P < 0,05; OR = 1,63; 95% CI = 1,07–2,49).V disertační práci jsme prokázali vztah genové variability interleukinu-10, NOD-like receptoru 3 a serotoninového transportéru k manifestaci RAS. Zařazení těchto asociovaných polymorfizmů do etiopatogenetických drah RAS potvrdilo možný vliv Th1/Th2 nerovnováhy, zvýšené permeability orální sliznice a snížené orální tolerance na výsev aftózních lézí.

Abstract:

Recurrent aphthous stomatitis (RAS) is a multifactorial inflammatory disease of the oral cavity characterized by the eruption of painful erosions or ulcers on the oral mucosa. The etiopathogenesis of RAS has not been elucidated yet, but genetic factors play an important role in its development. We conducted a literature search of the genetic background of RAS and the analysis of selected variants in genes for interleukins, matrix metalloproteinases, NOD-like receptor 3 and serotonin transporter in patients with RAS and healthy controls in the Czech population within the dissertation thesis. The dissertation thesis is centred on six published papers accompanied by comments by the author. These articles comprise of one literature review describing genetic predisposition to RAS and five case-control genetic association studies. Methods based on polymerase chain reaction (PCR) were used to determine single nucleotide polymorphisms (SNPs) and variable number tandem repeats (VNTRs). Out of the 19 genes studied, only the genes for interleukin-10 (IL10), NOD-like receptor 3 (NLRP3) and serotonin transporter (SLC6A4) were associated with RAS. In the IL10 gene, we evaluated T allele and TT genotype of SNP IL10 -819C/T (P < 0.05; OR = 1.63; 95% CI = 1.07–2.46 and P < 0.05; OR = 2.93; 95% CI = 1.19–7.24, respectively) and A allele and AA genotype of SNP IL10 -592C/A as risk variants for RAS (P < 0.05; OR = 1.73; 95% CI = 1.14–2.62 and P < 0.05; OR = 2.93; 95% CI = 1.19–7.24, respectively). Haplotype analysis determined haplotype IL10 ACC (-1082A/G, -819C/T, -592C/A) as protective (P < 0.05; OR = 0.58; 95% CI = 0.37–0.91). In contrast, haplotype IL10 ATA (-1082A/G, -819C/T, -592C/A) was statistically significantly more frequent in RAS patients (P < 0.05; OR = 1.63; 95% CI = 1.07–2.50). Also, the TT genotype of NLRP3 rs4612666 polymorphism was more common in RAS patients in comparison with CT + CC genotypes (P < 0.05; OR = 14.69; 95% CI = 1.73–124.72). We found an association between the LA12 haplotype of the SLC6A4 gene (5-HTTLPR, rs25531, STin2) and increased risk of RAS (P < 0.05; OR = 1.63; 95% CI = 1.07–2.49). In this dissertation thesis, we have demonstrated the relation of gene variability in interleukin-10, NOD-like receptor 3 and serotonin transporter to RAS manifestation. Incorporation of these associated polymorphisms into the etiopathogenetic pathways of RAS confirmed the possible influence of Th1/Th2 imbalance, increased oral mucosal permeability and decreased oral tolerance on the occurrence of aphthous lesions.
