Abstract:

Cíl: Cílem práce bylo zjistit postoj porodních asistentek a dalších zdravotnických, psychologických a sociálních pracovníků běžně poskytujících služby ženě v období těhotenství, porodu a šestinedělí, jakožto i této samotné ženy, k práci porodní asistentky v komunitním prostředí. Metodika: Výzkumný soubor tvořilo 13 odbornic z celé ČR a 8 vysokoškolsky vzdělaných žen (7 matek a 1 poprvé těhotná) z jižních Čech. Výzkum (červenec 2012 - leden 2013) byl veden formou individuálních polostandardizovaných rozhovorů. Výsledky: Šetřením bylo zjištěno, že dotázané považují porodní asistentku za kompetentní v poskytování individuální kontinuální péče ženě a dítěti v jejich přirozeném prostředí, ale také např. v porodních domech, které jsou běžnou součástí nabídky porodní péče v zahraničí, ale u nás je zatím nemáme. Žádná z dotazovaných odbornic necítí v porodní asistentce konkurentku, všechny si ale zároveň uvědomují, že většina gynekologů brání svůj status a ekonomické zájmy, což má za následek zbytečné snížení spolupráce mezi gynekologem a porodní asistentkou, které je jen na úkor ženy. Dotazované matky popisují pozitivní zkušenosti se svými komunitními porodními asistentkami, ačkoliv jejich počet je velmi omezený a bylo poměrně obtížné je k asistenci u domácího porodu zajistit. Závěr: Obě skupiny respondentek vnímají současnou situaci s absencí systémové a legislativní podpory jako nepříznivou. Přesto vidí v péči o ženu komunitní porodní asistentkou mnoho výhod a přiklánějí se k tomu, aby se komunitní porodní asistence do budoucna dále rozvíjela.

Abstract:

Objective: The objective of this thesis was to find out about attitudes of midwifes and other medical, psychological and social workers normally providing health care services to a female (incl. her attitudes) during pregnancy, childbirth and the puerperium towards the work of midwife in the community environment. Methodology: The survey sample consisted of 13 female specialists across the Czech Republic and 8 female graduates (7 mothers, 1 primipara) across the South Bohemia. The survey itself (July 2012 - January 2013) was made by means of individual half-standardizes interviews. Results: The survey showed the questioned persons regarded midwife as a competent provider in terms of continual individual looking after woman and her child (not only) in their natural environment, but e. g. also in birth centres that are taken for a regular part - in contrast to their absence in the Czech Republic - in foreign childbirth care systems. None of the questioned specialists perceives midwives as a competition. Nevertheless, all the specialists realize at the same time that most of gynaecologists defend their status and economic interests which results in unnecessary reduction of the cooperation between both the gynaecologist and the midwife. That is logically at the expense of a pregnant woman. Questioned mothers describe positive experiences with their midwives, although the number of them is very limited and it has been quite difficult to get one to assist in the childbirth at home. Conclusion: Both of the respondent groups perceive the current situation that reflects the absence of any systemic and legislative support as inconvenient. Yet they find many assets in the care of a woman by the community midwife and they incline to support the development of community midwifery in future.
