Abstract:

Bakalářská práce se zaměřuje na téma validace diagnostických prvků u ošetřovatelské diagnózy: Neefektivní kojení 00104 z pohledu klasifikačního systému NANDA, který nabízí mnoho možností pro zlepšení a zkvalitnění ošetřovatelské péče. Zaměřuji se na ověření míry platnosti ošetřovatelské terminologie NANDA pro péči o kojení v českých podmínkách. Hlavním cílem bakalářské práce je zjistit, zda je ošetřovatelská diagnóza Neefektivní kojení NANDA validní. Teoretická část se zabývá kojením, laktací a popisem klasifikačního systému NANDA. Praktická část se zaměřuje na to, jaká je platnost ošetřovatelské terminologie NANDA v péči o kojení u nás. Výzkum byl proveden v Masarykově nemocnici v Ústí nad Labem na oddělení šestinedělí. K výzkumu bylo použito anonymní dotazníkové šetření, kterého se dohromady zúčastnilo 26 porodních asistentek a dětských sester s minimálně 5letou praxí. Jako měřící nástroj byly použity diagnostické prvky, které jsou použity v klasifikačním systému NANDA International. Expertky považují za diagnosticky významný jen jeden určující znak a to kojenec není schopen správně uchopit prs s váženým průměrem 0,82. 5 souvisejících faktorů je dle expertek diagnosticky významných. Mezi diagnosticky významné související faktory řadí: anomálie prsů matky (vážený průměr 0,85), úzkost matky (vážený průměr 0,85), předčasně narozené dítě (vážený průměr 0,83), slabý sací reflex kojence (vážený průměr 0,82) a předchozí neúspěšné pokusy v kojení (vážený průměr 0,81). Ostatní určující znaky a související faktory jsou diagnosticky méně významné. Pro zpestření bakalářské práce jsem ještě uskutečnila druhé anonymní dotazníkové šetření, kterého se zúčastnilo 30 matek hospitalizovaných na oddělení šestinedělí v Masarykově nemocnici v Ústí nad Labem. Cílem bylo zjistit informovanost matek o kojení. Při vyhodnocování dotazníkového šetření byly zjištěny značné rozdíly v odpovědích matek a sester expertek. Dle výsledků je zřejmé, že edukace o kojení a péče, kterou poskytuje personál v průběhu hospitalizace na oddělení šestinedělí, je nedostačující.

Abstract:

Bachelor thesis focuses on the topic of validation of diagnostic elements of a nursing diagnosis: Ineffective breastfeeding 00104 from the perspective of the classification system of NANDA, which offers many opportunities for improvement and improving the quality of nursing care. I focus on the verification of the extent of validity of the nursing terminology of NANDA for the care of breastfeeding in the Czech republic. The main aim of this thesis is to find out whether it is nursing diagnosis Ineffective breastfeeding NANDA valid. The theoretical part deals with breastfeeding, lactation and a description of the classification system of NANDA. The practical part focuses on what is the validity of nursing terminology of NANDA in the care of breastfeeding with us. The research was conducted at the Masaryk hospital in Ústí nad Labem on the department of the postpartum period. The research was used an anonymous questionnaire survey, which together was attended by 26 midwives and paediatric nurses with a minimum of 5 years practice. As a measuring tool were used the diagnostic elements, which are used in the classification system NANDA International. Experts considered diagnostically significant only one defining character and that the infant is not able to properly grasp the breast with a weighted average of 0,82. 5 related factors, according to experts diagnostically significant. Between the diagnostically significant related factors include: anomalies of the breasts of the mother (the weighted average of 0,85), the mother's anxiety (a weighted average of 0,85), premature baby (a weighted average of 0,83), weak sucking reflex of the infant (the weighted average is 0,82) and previous failed attempts at breastfeeding (a weighted average of 0,81). Other defining characteristics and related factors are diagnostically less important. To diversify the bachelor's work I've carried out second anonymous survey, which was attended by 30 mothers of hospitalized at the department of the postpartum period in Masaryk hospital in Ústí nad Labem. The aim of was to find out awareness of mothers about breastfeeding. In the evaluation of the questionnaire survey revealed significant differences in the responses of mothers and of nurses experts. According to the results it is clear that education about breastfeeding and care provided by staff during hospitalization at the department of the postpartum period, is insufficient.
