Monitoring of microbial colonization in ICU patients

Abstract:

Kriticky nemocní pacienti na odděleních JIP jsou neustále ohrožováni infekcemi způsobenými polyrezistentními nemocničními kmeny. Tyto kmeny jsou obvykle potenciálně patogenní a vyvolávají tzv. nozokomiální infekce. I v dnešní době je velkým problémem vznik výrazné a poměrně časté rezistence mikrobů k základním antibiotikům a léčba se tak velmi komplikuje. Ve své práci jsem se zaměřila na popis jednotlivých kmenů, diagnostiku gramnegativních bakterií s produkcí širokospektrých beta-laktamáz (ESBL), methicilin rezistentních zlatých stafylokoků (MRSA), vankomycin rezistentních enterokoků (VRE) a zjištění četnosti výskytu polyrezistentních kmenů u pacientů na Klinice anesteziologie, resuscitace a intenzivní medicíny ve Fakultní nemocnici Bohunice v Brně. Má práce mě dovedla k závěru, že i přes veškerá preventivní a hygienická opatření jsou kolonizace polyrezistentními kmeny a nozokomiální nákazy stále časté. Je nutné věnovat pozornost mimo jiné také antibiotické léčbě, která by měla být volena pečlivě s ohledem na konkrétní zdravotní stav pacienta tak, aby ještě více nezatížila jeho organismus.

Abstract:

Patients with critical illnesses placed in the ICU (Intensive-care unit) are being continually endangered by the infections caused by multidrug-resistant hospital bacteria. These bacteria are usually potentially pathogenic and provoke so called nosocomial infections (also known as hospital-acquired infections). Even today in the modern society the occurrence of prominent and rather common resistance of the microbes to basic antibiotics is a serious problem that significantly prolongs and complicates the therapy. In my thesis I've focused on the bacteria species, diagnosis of gram-negative bacteria with production of broad-spectrum beta-lactamase (ESBL), methicillin-resistant Staphylococcus aureus (MRSA), vancomycin-resistant enterococcus (VRE) and especially establishing the frequency of appearance of the multidrug-resistant hospital bacteria at the patients from Department of Anesthesiology and Intensive Care, The University Hospital Bohunice in Brno. My thesis has led me to an assumption that despite all the preventive and hygienic actions the colonisation by multidrug-resistant hospital bacteria and nosocomial infections are still very common. It is also necessary to focus on the antibiotic treatment that should be adjusted cautiously respecting the particular state of health of the patient so that the treatment would not burden the organism.
