Trauma in the anamnesis of patients with borderline personality disorder and their coping strategies.

Abstract:

Vědecké práce se stále zabývají etiologií hraniční poruchy osobnosti, její diagnostikou a následnou léčbou. V zájmu pozornosti je i chování jedinců s hraniční poruchou osobnosti a s tím spojené problémy v regulaci emocí a v mezilidských (interpersonálních) vztazích. Tato diplomová práce je zaměřena na tematiku hraniční poruchy osobnosti a copingové strategie jedinců s touto poruchou. Prvním cílem této studie bylo zjistit, jaká traumatická událost se nachází v anamnéze pacientů s hraniční poruchou osobnosti. Druhým cílem studie bylo zjistit, jaké copingové strategie při zvládání stresové situace využívali pacienti s hraniční poruchou osobnosti v minulosti a jaké využívají nyní v přítomnosti. Výzkumný soubor se skládal z 5 respondentů (5 žen). Průměrný věk ve skupině respondentek byl 28,2 let, věkové rozmezí bylo mezi 19 až 38 lety. Nejvyšší dosažené vzdělání respondentek se pohybovalo od středoškolského až po vysoko-školské. Sběr dat proběhl formou polostrukturovaných rozhovorů, které byly poté následně doslovně přepsány. Respondentky byly osloveny k zapojení do výzkumu náhodně. Hlavními tématy rozhovoru tedy byly trauma, posttraumatická stresová porucha, hraniční porucha osobnosti (její rysy) a copingové strategie. Analýza dat (rozhovorů) byla provedena pomocí IPA (interpretativní fenomenologické analýzy). Výsledky ukázaly na přítomnost traumatických událostí v anamnéze respondentek. Konkrétně se objevilo domácí násilí, šikana, úmrtí v rodině a znásilnění. Mezi nejčastější identifikované maladaptivní copingové strategie patří sebepoškozování, abúzus návykových látek (alkohol, drogy nebo nadužívání psychofarmak) a předstírání, že se situace nestala (neděje). Z adaptivní copingových strategií respondentky zmiňují oporu v rodině i přátelích (mohou si dojít pro radu i emoční podporu) a přesunutí pozornosti na jinou činnost (záliba apod.). Výsledky ukázaly převahu maladaptivních copingových strategií při traumatické události v dětství (které lze označit v anamnéze jako trauma). U některých respondentek došlo ke změně maladaptivních strategií na adaptivní. Příčinu této změny nelze zobecnit pouze na základě zjištění této studie.

Abstract:

Scientific papers are still investigating the etiology of borderline personality disorder, her diagnosis and subsequent treatment. Also in the interest of attention is the behaviour of individuals with borderline personality disorder and the associated problems in emotion regulation and in interpersonal relationships. This thesis focused on the topic of borderline personality disorder and coping strategies of individuals with this disorder. The first aim of this study was to determine what traumatic event there was in the history of patients with borderline personality disorder. The second aim of the study was to find out what coping strategies had been used by patients with borderline personality disorder in a particular stress situation in the past and what they were using in the present. The research set consists of 5 respondents (5 women). The average age in the group was 28.2 years, the age range was between 19 and 38 years. Respondents highest educational attainment ranged from high school to college. The data collected the form of semistructured interviews, which were transcribed. Respondents were approached about engaging in research randomly. The main topics of interviews were trauma, posttraumatic stress disorder, borderline personality disorder and coping strategies. Data analysis (interviews) was done using IPA (Interpretative Phenomenological Analysis). The results showed the presence of traumatic events in female respondents. There was domestic violence, bullying, family deaths and rape. Self-harm is among the most common maladaptive coping strategies identified, substance abuse (alcohol, drugs or psychopharmaceuticals) and pretending the situation did not happen (not happening). Among the adaptive coping strategies, respondents mention support in family and friends (they can go for advice as well as emotional support) and shifting focus to another activity (hobbies, etc.). The results showed a preponderance of maladaptive coping strategies in a traumatic event in childhood (a history of trauma may be noted). Some respondents experienced a change in maladaptive strategies to adaptive ones. The cause of this change cannot be generalized based on the findings of this study alone.
