Quality of life in children with Cerebral Palsy

Abstract:

Diplomová práce se zabývá kvalitou života dětí s dětskou mozkovou obrnou (DMO). Hlavním cílem práce je překlad a validace dotazníku CPCHILD (Caregiver Priorities and Child Health Index of Life with Disabilities) do českého jazyka a prostředí. Dílčím cílem práce je aplikace daného dotazníku na vybranou skupinu dětí s DMO ve věku od 2 do 18 let. Tedy pokračování procesu validace a za pomocí pre-testu následné hodnocení kvality života dětí s DMO v kontextu klasifikace hrubé motoriky GMFCS (Gross Motor Function Classification System). Proces překladu a validace dotazníku CPCHILD vymezuje samostatný výzkumný úkol. Na jehož konci byl přeložený dotazník aplikován do praxe skrze online anonymní dotazníkové šetření. Nejdříve formou pilotáže, poté pre-testu a pro ověření reliability i testu retestu. Proběhlo ověření reliability (test-retest, ICC, Cronbachova ?) a validity (Zjevná validita, a Known-Groups Validita) české verze dotazníku. Celkem bylo získáno 79 vyplněných dotazníků od pečovatelů dětí s DMO ve věku od 3 do 18 let v rámci celé České republiky. Dle výsledků hodnocení kvality života byl shledán statisticky významný rozdíl mezi hodnotou celkového skóre dotazníku CPCHILD a stupněm klasifikace hrubé motoriky GMFCS. Existující negativní korelace (r= -0,77; p<00001) značí, že jedinci s vyšším (horším) stupněm GMFCS mají nižší (horší) celkové skóre dotazníku CPCHILD. Děti s horším stupněm GMFCS mají horší skóre dotazníku, a tedy horší kvalitu života. Česká verze dotazníku CPCHILD je na základě testů považována jako validní a reliabilní forma nástroje pro hodnocení kvality života dětí s DMO.

Abstract:

This diploma thesis deals with the quality of life in children with cerebral palsy. The main goal of this thesis was Translation and Validation of the CPCHILD Questionnaire (Caregiver Priorities and Child Health Index of Life with Disabilities) into the Czech language and cultural context as a trans-cultural adaptation. A partial goal of the work was the application of a translated questionnaire to a selected group of children with Cerebral Palsy in age between 2 to 18 years. Thus, the continuation of the validation process and as a pre-test to evaluate the Quality of Life of these children with Cerebral Palsy in the context of the Gross Motor Function Classification System (GMFCS). The process of translation and validation of the CPCHILD questionnaire is a separate research task. At the end of which the translated questionnaire, now in Czech language was applied in practice through an online anonymous questionnaire survey. First in the form of pilot study, then pre-test and then test-retest. The reliability (test-retest, ICC, Cronbach's ?) and validity (Face validity, and Known-Groups Validity) of the Czech version of the questionnaire were tested. A total of 79 responses were obtained from caregivers of children aged 3 to 18 throughout the Czech Republic. According to the results of the Quality of Life evaluation, a statistically significant difference was found between the value of the overall total score of the CPCHILD questionnaire and the degree of GMFCS. The existing negative correlation (r = -0.77; p <00001) indicate that individuals with a higher (poorer function) GMFCS levels have a lower (worse) overall total CPCHILD score. Children with a worse level of GMFCS have a worse overall total score of the CPCHILD questionnaire and therefore have a worse Quality of Life. Based on tests, the Czech version of the CPCHILD questionnaire is considered a valid and reliable form as a tool for assessing the quality of life in children with Cerebral Palsy
