Neurological aspects of degenerative changes in the lumbar spine

Abstract:

Tato dizertační práce se primárně zabývá analýzou degenerativních změn v oblasti bederní páteře, jejich klinickou manifestací (především bolestí dolní části zad, včetně její neuropatické komponenty), a hledáním optimálních diagnostických možností a rizikových faktorů pro neuropatickou bolest u chronických syndromů spojených s bolestí dolní části zad. Formou přehledových článků tato práce také poukazuje na strukturální a funkční změny lumbálních paraspinálních svalů u pacientů s chronickými nespecifickými bolestmi dolní části zad a jejich roli v patogenezi bolesti, dále upozorňuje na diskrepanci mezi radiologickými a klinickými nálezy u pacientů s lumbální spinální stenózou a shrnuje problematiku bolestí dolní části zad ve stáří. Klíčové výstupy publikovaných studií komentovaných v rámci této práce jsou následující: Neuropatická komponenta bolesti je často přítomna u pacientů s chronickým radikulárním syndromem, což bylo potvrzeno ztrátou percepce senzitivních modalit pomocí QST. Dotazník PainDETECT s cut-off hodnotou 19 identifikoval neuropatickou komponentu u poměrně malé části pacientů s chronickým radikulárním syndromem. Chronický radikulární syndrom může být spojen se snížením hustoty intraepidermálních nervových vláken. Degenerativní změny v oblasti bederní páteře jsou častým nálezem na magnetické rezonanci u pacientů s chronickou nespecifickou bolestí dolní části zad, tyto změny ale nekorelují s intenzitou bolesti a nepredikují neuropatickou bolest. Jedinými nezávislými prediktory neuropatické bolesti u pacientů s chronickou nespecifickou bolestí dolní části zad byly ženské pohlaví a intenzita bolesti. Chronická nespecifická bolest dolní části zad je spojena se strukturálními a funkčními změnami lumbálních paraspinálních svalů, ale vztah mezi bolestí, degenerací paraspinálních svalů a degenerací páteře musí být ještě objasněn v dalším výzkumu.

Abstract:

The aim of this doctoral thesis is to analyse the degenerative changes in the lumbar spine, their relationship with clinical manifestations (in particular low back pain, including its neuropathic pain component), and the search for optimal diagnostics and risk factors for neuropathic pain in chronic low back pain syndromes. In review articles, this thesis also refers to the structural and functional changes of the lumbar paraspinal muscles in patients with chronic nonspecific low back pain and their role in the pathogenesis of pain, highlights the discrepancy between radiological and clinical findings in lumbar spinal stenosis, and summarises the issue of low back pain in the elderly. Key findings from the published commented studies are the following: A neuropathic component of pain is frequently present in patients with chronic radicular syndrome, which appears to be confirmed by sensory loss documented by quantitative sensory testing. Using a cut-off score of 19, the PainDETECT questionnaire identified a neuropathic pain component in a comparatively small proportion of patients with chronic radicular syndrome. Chronic radicular syndrome may be associated with a reduction in intraepidermal nerve fibre density. Degenerative changes in the lumbar spine are common magnetic resonance imaging findings in patients with chronic nonspecific low back pain, but they do not correlate with pain intensity and do not predict neuropathic pain. The only independent predictors of neuropathic pain in patients with chronic nonspecific low back pain were female sex and the pain intensity. Chronic nonspecific low back pain is associated with structural and functional changes in the lumbar paraspinal muscles, but the relationship between pain, paraspinal muscle degeneration, and spinal degeneration needs to be clarified by further research.
