DNS method in patient with skoliosis

Abstract:

Úvod: Úvodem bakalářské práce bych ráda poukázala na problematiku skolióz a možnost léčby v rámci fyzioterapie. Současně se v práci zaměřuji na metodu Dynamické neuromuskulární stabilizace a její význam v terapii u dané diagnózy. Skolióza se řadí mezi problematiku fyziologického zakřivení páteře jedince, které často může vznikat, zejména v dnešní době, ze sedavého zaměstnání, neaktivního životního stylu dětí, mladistvých a dospělí. Etiologie této diagnózy je nejvíce v dětském věku či dospívání. Důsledky nám mohou ovlivnit celé tělo od pohybového až k viscerálnímu systému. Metoda DNS slouží převážně k posílení hlubokého stabilizačního systému a centrovaného postavení kloubů. V této práci bych chtěla poukázat na možnost využití této metody a její průkaznosti u dané diagnózy. Cíl práce: Cílem této práce je průkaznost metody dynamické neuromuskulární stabilizace u diagnózy skolióz. Tímto bych chtěla přispět k řešení této problematiky a zvýšení možnosti začlenění metody DNS do terapie u této diagnózy. Metodika: V rámci práce jsem vypracovala kazuistiku u osmnáctiletého muže diagnostikovaného pro juvenilní idiopatickou skoliózu. Dle vstupního vyšetření jsem vytvořila krátkodobý fyzioterapeutický plán. Tento plán se skládal z jednotlivých konceptů. V terapiích jsem využila měkké a myofasciální techniky, postizometrickou relaxaci, postfacilitační strečink, mobilizace, bazální posturální program dle Čápové a Dynamickou neuromuskulární stabilizaci. V průběhu terapie jsem provedla kontrolní kineziologické vyšetření. V rámci poslední terapie jsem provedla výstupní kineziologické vyšetření. Výsledky: Z výstupního vyšetření bylo znatelné zlepšení všech měřených hodnot. Držení a konfigurace poukazuje na zlepšení především v oblasti držení ramenních kloubů, u kterých je zmenšení protrakčního držení. Dále došlo ke zlepšení amplitudy hrudníku, obvodových měr přes proccesus xiphoideus. Střední postavení hrudníku, nádech, výdech a pružnost hrudníku byla zvětšena. Měkké tkáně a facie neprokazují patologie. V terapiích jsem odstranila kloubní blokádu SI skloubení. Dále došlo k uvolnění hypertonických svalů v oblasti m. trapezius, m. pectoralis major oboustranně, m. levator scapulae a zmírnění paravertebrálních svalů. Krční páteř je v plném rozsahu pohybu. Pohyblivost páteře byla zlepšena ve všech měřeních. Dále také došlo k optimalizaci svalové síly převážně v oblasti krku a trupu. Ostatní testy dále vykazovaly zlepšení. Těmito testy jsou vyšetření olovnicí, zkoušky vah a test dle Rhomberga. Z testů posturální stabilizace a posturální reaktivity bylo zlepšení ve všech pozicích. Stav bez patologie je u bráničního testu, testu nitrobřišního tlaku a testu flexe hlavy a trupu. Nejvýznamnějším výsledkem této práce byla korekce skoliotické křivky dle RTG vyšetření v C/TH z 18 st. na 16 st., střední a dolní poloviny TH páteře z 24 st. na 22 st. a TH/L oblasti z 15 st. na 13 st. Závěry: Díky využití výše zmíněných postupů došlo ke zlepšení stavu pacienta, pohyblivosti páteře, amplitudy hrudníku, obvodových rozměrů přes proccesus xiphoideus, středního postavení hrudníku, nádechového a výdechového postavení a pružnosti hrudníku. Dále došlo ke zlepšení držení a konfigurace, svalové síly. Z terapií vyplývá zlepšení v testech posturální stabilizace a posturální reaktivity a snížení skoliotické křivky u C/TH o 2 st., střední a dolní poloviny TH o 2 st. a TH/L přechodu o 2 st. Pro udržení stavu pacienta je důležité pravidelné a důsledné cvičení edukovaných poloh dynamické neuromuskulární stabilizace a přídatných autoterapií.

Abstract:

Introduction: At the beginning of the bachelor thesis I would like to point out the issue of scoliosis and the possibility of treatment in physiotherapy. I focus on the method of Dynamic Neuromuscularr Stabilization at the same time and its importance in therapy for a given diagnosis. Nowadays is scoliosis one of the problems of physiological curvature of the spine, which can often arise from sedentary work, inactive lifestyle of children, adolescents and adults. The etiology of the diagnosis is mostly in childhood or adolescence. The consequences can affect the whole body from the musculoskeletal to the visceral system. The DNS method is mainly used to strengthen the deep stabilization system and the centered position of the joints. In this thesis, I would like to point out the possibility of using this method and its evidence in the diagnosis. Aim of the work: The aim of this thesis is to prove the method of the Dynamic Neuromuscular Stabilization in the diagnosis of scoliosis. I would like to contribute to solving this problem and increase the possibility of including the DNS method in the therapy of this diagnosis. Methodology: In this work, I developed a case report of a 18 year old man diagnosed with juvenile idiopathic scoliosis. I created a short-term physiotherapy plan according to the initial examination. This plan was consisted of individual concepts. For therapies I used soft and myofascial techniques, postisometric relaxation, postphacilitic stretching, mobilization, basal postural program according to Čápová and Dynamic Neuromuscular Stabilization method. I performed a control kinesiological examination during the therapy. As part of the last therapy, I performed a final kinesiological examination. Results: There was a noticeable improvement of all measured values from the final examination. Posture and body configuration indicate improvements especially in the area of the shoulder joints, where there is a reduction in protraction posture. There was also an improvement in chest amplitude, circumferential measures through the process xiphoid. Mean chest position, inspiration, expiration, and chest flexibility were increased. Soft tissues and fascia do not show pathology. I removed the joint blockage of the SI joint in the therapies. Furthermore, the hypertonic muscles in the area of the trapezius muscle, the pectoralis major muscle bilaterally, the levator scapulae muscle were relaxed and the paravertebral muscles were relaxed. The cervical spine is in full range of motion. Spinal mobility was improved in all measurements. Muscle strength was also optimized, mainly in the neck and torso. Other tests also showed improvements. These tests are the lead test, scale test and Rhomberg test. Postural stabilization and postural reactivity tests showed improvement in all positions. The pathology-free condition is in the diaphragm test, the intra-abdominal pressure test and the head and torso flexion test. The most significant result of this work was the correction of the scoliotic curve according to X-ray examination in C / TH from the 18 deg. to 16 deg., middle and lower halve of TH spine from 24 deg. to 22 deg. and TH / L areas from 15 deg. to 13 deg. Conclusions: The use of the above procedures improved the patient's condition, spinal mobility, chest amplitude, circumferential dimensions through the xiphoid process, middle chest position, inspiratory and expiratory position and chest flexibility. There was also an improvement in posture and muscle strength. Thanks to the therapies, there were improvements in postural stabilization and postural reactivity tests and a reduction in the scoliotic curve for C / TH by 2 deg., the middle and lower halves of TH by 2 deg., and the TH / L transition by 2 deg. To maintain the patient's condition, regular and consistent exercise of the educated positions of dynamic neuromuscular stabilization and additional autotherapies is important.
