Nursing Process in Care of Patient after Kidney Transplantation

Abstract:

Cílem této bakalářské práce je zpracování ošetřovatelského procesu u pacienta po transplantaci ledviny, který vychází z ošetřovatelských diagnóz NANDA I. taxonomie II. Ošetřovatelské diagnózy se vztahují ke konkrétním potřebám vybraného jedince. Práce je rozdělena na teoretickou a praktickou část. Teoretická část je rozdělena do dvou kapitol. První kapitola se zabývá historii transplantací, indikací a kontraindikací k transplantaci, dárcovstvím a náhradami funkce ledvin. Druhá kapitola se zabývá ošetřovatelskou péčí o pacienta před, během a po transplantaci ledviny. Praktická část je zaměřena na vybraného pacienta před i po transplantaci ledviny. V předoperačním období jsou stanoveny tři ošetřovatelské diagnózy. Tyto diagnózy jsou stanovené na základě odebrané anamnézy, určujících znaků a souvisejících faktorů. Ke každé diagnóze byl stanoven cíl, intervence, realizace a následně hodnocení ošetřovatelské péče. V pooperačním období bylo stanoveno třináct diagnóz a taktéž byly stanoveny cíle, intervence a hodnocení realizace poskytované péče. Jednotlivé ošetřovatelské diagnózy jsou stanoveny dle NANDA I taxonomie II.

Abstract:

The goal of this bachelor thesis is to create an illustrative plan for nursing of patients after kidney transplantation that is based on the nursing diagnoses of NANDA I taxonomy II. The nursing diagnoses are applicable to the specific needs of the individuals. This work is divided in theoretical and practical parts. The theoretical part is divided into two chapters. The first chapter is about history, indications and contraindications of transplantation, donation and renal function replacement. The second chapter is focus on the patients nursing before, during and after transplantation. The practical part focuses on a chosen patient case before and after transplant. During the preoperative period three diagnosis are provided. These diagnosis are set based on the patients' medical history, defining characteristics and all related factors. Each of the diagnosis had an objective, intervention, realization and a final nursing care evaluation. In the after-operation stage were set thirteen diagnosis, each with their respective objective, intervention, realization and final nursing care evaluation. Each diagnosis was set in accordance to the NANDA I taxonomy II.
