iCMS characterization of colorectal cancer subtypes and their association with the gut microbiome

Abstract:

Kolorektální karcinom je nejen v České republice, ale i ve světě, jedno z nejčastěji diagnostikovaných maligních onemocnění. Kvůli jeho vysoké heterogenitě se jedná o onemocnění s komplikovanou léčbou a vysokou úmrtností. Z toho důvodu je snaha o nalezení co nejvhodnějších podtypů, které by usnadňovaly stratifikaci pacientů a volbu vhodné léčby. V dnešní době je již známo, že významný vliv na vznik a vývoj kolorektálního karcinomu může mít mimo jiné také střevní mikrobiom. Jedná se o společenství všech živých mikroorganismů v trávicím traktu živočichů a jejich genom. V této bakalářské práci se zabývám asociací vnitřních konsensuálních molekulárních podtypů s klinickými proměnnými, složením střevního mikrobiomu ze stěrů z nádoru, vzorků stolice a imunitního prostředí nádoru. Výsledky naznačují, že existují bakterie i imunitní buňky, které se mezi podtypy liší ve svém obsahu. Bakterie, které se mi podařilo asociovat s jednotlivými podtypy jsou v literatuře spojované s kolorektálním karcinomem. U podtypu iCMS2, který je spojován s lepší prognózou, jsem pozorovala bakterie, které jsou spojované s pozitivním efektem na zdraví pacientů, zatímco u podtypu iCMS3 s horší prognózou jsem pozorovala bakterie spojované s kancerogenezí.

Abstract:

Colorectal cancer is one of the most frequently diagnosed malignancies not only in the Czech Republic but also in the world. Due to its high heterogeneity, it is a disease with complicated treatment and high mortality. Therefore, efforts are made to find the most appropriate subtypes to facilitate patient stratification and choice of appropriate treatment. It is now known that the gut microbiome, among other factors, may have a significant impact on the development of colorectal cancer. This is the community of all living microorganisms in the digestive tract of individuals and their genomes. In this bachelor thesis, I investigate the association of internal consensus molecular subtypes with clinical variables, the composition of the gut microbiome from tumor swabs, stool samples, and the immune environment of the tumor. The results suggest that there are bacteria and immune cells that differ in amount between subtypes. The bacteria that I was able to link with each subtype are associated with colorectal cancer in the literature. For subtype iCMS2, which relates to a better prognosis, I observed bacteria associated with a positive effect on patient health, whereas for the subtype iCMS3 with a worse prognosis, I observed bacteria associated with carcinogenesis.
